Scope of Position:
Provides skilled care to patients in their residence under the direction of a RN and in accordance with the physician plan of care. Communicates and documents clinical finding as per agency policy and procedures. Functions with in the scope of practice for LVN and individual competencies.
Qualifications:
1. Licensed Vocational Nurse (LVN) licensed in the state of Texas.
2. One (1) year experience providing comprehensive skilled nursing care in the home health environment preferred.
Responsibilities/duties:
1. Furnishes skilled nursing services as assigned and in accordance the plan of care.
2. Prepares clinical and progress notes and submits to the agency according to agency protocols.
3. Accurately and effectively documents skilled observations and assessments, treatments and medications administered, patient/caregiver teaching, patient /caregiver response, and communication with physician and RN of findings and follow-up as appropriate.
4. Reports changes in patient status to RN and physician.
5. Updates the medication list with changes.
6. Documents physician orders and submits to RN for review and implementation of the change in plan of care.
7. Assists the physician and registered nurse in performing specialized procedures in accordance with the plan of care, LVN scope of practice, and individual competency.
8. Prepares equipment and materials for treatments observing aseptic technique as required to implement the plan of care.
9. Assists the patient in learning appropriate self-care techniques.
10. Participates in patient case conferences and staff meetings as directed.
11. Attends inservice and continuing education programs as requested.
12. Assist with orientation of new personnel as requested.
13. Participates in QAPI activities as requested.
14. Drives to and from patient residences to perform duties.
15. Performs other agency-related duties as assigned.
Job Type: Full-time
Pay: $22.00 - $30.00 per hour
