Description

Position Summary

The Care Programs Manager is responsible for the implementation, management, operation, supervision, follow-up, networking, coordination, and evaluation of all the Sanitas Disease management & Wellness programs. He/she works within the guidelines, policies and mission of the organization and will be accountable and responsible in reporting to the Executive leadership.

Essential Job Functions

  • Coordinate with the national leaders and regional program managers/leads as well as other stakeholders to create detailed implementation plans. This includes deadlines, milestones, processes, and risk mitigation protocols.
  • Initiate and set goals for Health Care programs based on the organization’s strategic objectives.
  • Collaborate with the regional Medical Directors on Disease Management Programs that serve the unique needs of their region.
  • Report detailed status updates to leadership and executive team quarterly and as requested.
  • Develop program operation workflows and budgets and present them to the Director, Value Based Optimization.
  • Define department job roles and responsibilities.
  • Provide leadership to all Care Programs and staff to achieve the organizational goals and objectives.
  • Share frequent feedback to the Care Programs team using the CASA framework.
  • Set annual objectives and goals and review clinical performance of the team and their respective program goals.
  • Build a strong team through open communication and by collaborating on decision-making responsibilities.
  • Collaborate with the Marketing team to create marketing initiatives that increase awareness of the programs provided.
  • Identify new and creative program ideas to implement through continuous research and participation in regional and national education conference offerings.
  • Ensure that the Health Care Programs deliver a return in outcomes and contribute to efficient cost management.
  • Collaborate with the payer in the delivery of joint program objectives.
  • Participate in payer meetings as necessary.
  • Support all LOBs and service line projects.
  • Align Programs to meet the needs of internal and external providers, for example PHSO Affiliates Providers.
  • All other duties assigned.

Supervisory Responsibilities

Carries out management responsibilities in accordance with the organization's policies. Manages and leads Care Program staff. Tasks include interviewing, hiring, and orienting employees; providing continuing educational opportunities; planning, assigning, and overseeing work; appraising performance; disciplining employees; scheduling; addressing complaints and resolving problems.

Physical Demands

The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job.

While performing the duties of this job the employee is regularly required to work standing up, walk, use hands to operate tools and equipment and must be able to exert regularly up to 10 pounds of force, frequently exert 30 pounds of force and occasionally exert 50 pounds of force to constantly perform the essential job functions. The employee will be frequently required to reach with hands and arms, bend, balance, kneel, crouch, crawl, push, and pull. Specific vision abilities required by this job include close vision, distance vision, peripheral vision, depth perception and ability to adjust focus. The employee is subject to environmental conditions, protection from weather conditions but not necessarily from temperature changes. The worker is subject to noise; there may be sufficient noise to cause the worker to shout to be heard above ambient noise level.

Qualifications


Required Education and Experience

  1. Proven track record as a successful program manager, or project manager.
  2. Experience managing teams and budgets.
  3. Excellent data analysis and reporting skills.
  4. Strong problem-solving, negotiation, and communication skills.
  5. Experience with industry-standard project management software and tools.
  6. Knowledge of performance evaluation techniques and key metrics.
  7. Must possess the skills to work in a physically and mentally demanding environment.
  8. Must understand the healthcare continuum and have ability to solve complex problems.
  9. Values consistent with culturally and family centered approach to care.
  10. Maintains patient confidentiality.
  11. Must be detail-oriented to ensure accuracy of reports and data.
  12. Ability to travel as required.

Preferred Education and Experience

  1. Bachelor of Nursing Degree preferred.
  2. 5 years Disease Management experience preferred.
  3. Exceptional leadership, time management, facilitation, and organizational skills.
  4. Proficient software computer skills to include WORD, EXCEL, and other Microsoft Office programs.
  5. Managed Care Experience preferred.
  6. Experience with electronic medical records (EMR), eClinicalWorks is a plus.
  7. Understanding of project management.
