Position Summary
This position is a Registered Nurse (RN) who performs patient health education at their assigned primary care facility through a unique care program that allow patients to achieve self-management. Together with a Care Promoter Medical Assistant (MA), and under the direction of the Lead Care Program Nurse, the Care Educator (RN), plans, coordinates, and holistically monitor patients’ needs to positively impact their chronic health condition through education. The Care Educator (RN) interacts with patients though a variety of mediums that include onsite face to face, telephonic encounters, and group workshops. The Care Educator (RN) might also possess clerical responsibilities of clinical supplies and vaccine inventory management. This position collaborates with a multi-disciplinary team to assure the highest quality care while focused on the physiological, psychosocial and safety needs of each patient and their families.

The employee will be frequently required to reach with hands and arms, bend, balance, kneel, crouch, crawl, push, and pull. Specific vision abilities required by this job include close vision, distance vision, peripheral vision, depth perception and ability to adjust focus. The employee is subject to environmental conditions; protection from weather conditions but not necessarily from temperature changes. The worker is subject to noise; there may be sufficient noise to cause the worker to shout in order to be heard above ambient noise level.
