Position Purpose:The Behavioral health case manager lead reports to the Associated director of BH program. This role is a core member of the Sanitas Medical Centers care team and is responsible for coordinating the day-to-day activities of the behavioral health case management program, assisting clinicians in developing clinical skills, maintaining an appropriate patient-clinician ratio, and ensure compliance of all clinical protocols.
Summary of Duties:
Ensure that behavioral health case manager maintain professional practice standards for group and/or individual therapeutic modalities.
Support both new and existing program staff by providing field training, perspective, and empowerment to enable direct service personnel to effectively deliver services.
Offer consultation to student interns and non-behavioral health care staff regarding behavioral health care services, programs, and guidelines.
Conduct regular clinical audits and implement employee improvement plans as needed.
Ensures compliance with related federal, state and local regulations, as well as departmental and county policies and procedures.
Participates in the establishment or revision of policies, procedures, guidelines, goals and objectives.
Supervises staff in assessing social, emotional, and economic causes of problem behavior and mental or emotional disorders, developing and modifying care plans, termination or transfer of cases.
Provide support to program staff through regular individual and group staff meetings.
Offer assistance in crisis or problematic cases and ongoing support to program staff in coordinating services for patients, including after-hours assistance.
Serve as a resource to direct reports in resolving complaints and reporting incidents
Serve as liaison and point of contact for all case management and related outreach events for the Behavioral Health Department.
Prepare weekly/ monthly/quarterly, annual statistical reports, and other operational reports as required.
Ensure quality, integrity and continuous improvement of organizational programs
Develops departmental clinical expertise to enhance overall healthcare delivery.
Maintains an open line of communication with all Clinical Supervisors, Operations Supervisor, and Program Director.
Promotes teamwork among staff.
Performs other duties as assigned
Supervisory Responsibilities: This position manages assigned employees of the department and is responsible for the performance management and hiring of the employees within the department.
Education and Experience
Masterdegree in social work or any related field
Certification: Certified Addiction Counselor (CAC)
BLS Certification required
Job Experience (years) 3-5 years of experience
BLS certification (required)
License: Active Florida license in clinical social work (LCSW), mental health counseling (LMHC/LPC)
Other Knowledge, Skills and Ability
Relevant experience or any other job-related vocational course on brief psychotherapy techniques
Flexible schedule weekdays and weekends
Ability to travel to an assigned region
Excellent oral and written communication skills
Ability to show empathy and compassion
Able to establish and maintain effective working relationships with mentally or emotionally disabled persons and their families.

Physical Demands: While performing the duties of this job the employee is regularly required to work standing up, walk, use hands to operate tools and equipment and must be able to exert regularly up to 10 pounds of force, frequently exert 30 pounds of force and occasionally exert 50 pounds of force to constantly perform the essential job functions. The employee will be frequently required to reach with hands and arms, bend, balance, kneel, crouch, crawl, push, and pull. Specific vision abilities required by this job include close vision, distance vision, peripheral vision, depth perception and ability to adjust focus. The employee is subject to environmental conditions; protection from weather conditions but not necessarily from temperature changes. The worker is subject to noise; there may be sufficient noise to cause the worker to shout in order to be heard above ambient noise level.
Employees are required to wear a mask in common areas. In addition, employees must comply with any state and local masking order.
Organizational Relations:
Internal : Associatedirector Behavioral health counselor - Purpose:Works in tandem with the BH case manager supervisor to staff cases enrolled in the program, maintain program protocol and ensure patient satisfaction Frequency: Weekly.
External:Care coordination and disease management programs Purpose:Work in collaboration with care coordinators and nursing management team to ensure seamless transition of care between programs. Frequency: Monthly
