Sangre de Cristo Community Care is looking for a Part Time Bachelor’s Level (Master’s preferred) Social Worker to join our dynamic team in our Pueblo, CO office. Responsibilities:

· Provides psychosocial support to patients and families by provided skilled social work invention

· Provides resources, referrals, education, supportive counseling, discharge planning and assistance with final arrangements

· Works closely with the inter disciplinary team to provide a continuous appropriate, comprehensive, and responsive plan of care for patients.

Requirements

• A Bachelor’s Degree in social work, counseling, or related field from an accredited university (Master’s preferred)
• Must have a valid non-restricted driver’s license
• Bilingual in Spanish preferred
• Flexibility and availability to work a weekend schedule if needed.
• At least 1 year of healthcare experience preferred.
• Participation in the seasonal Coronavirus Disease 2019 (COVID-19) Vaccination program is a requirement for all Sangre employees
