Responsibilities:

Will be under the indirect supervision of the registered nurse, advanced practice registered nurse, licensed physician (MD or DO), or other health care provider authorized by the state; will participate in the development, implementation, and modification of the comprehensive plan of care for facility residents. Will practice within the legal boundaries authorized by the state for the delivery of patient care and related service.

  • Practices under the indirect supervision of a licensed Registered Nurse (RN) from the applicable state, who is available onsite throughout the entire shift.
  • Reports directly to the individual providing indirect supervision by written or verbal instructions and face-to-face communication.
  • Adhere to state rules and regulations concerning the delivery of care and assures that effective quality nursing care is delivered which is outcome focused through utilization of the nursing process.
  • Effectively interview the resident/responsible party to obtain information required for developing and implementing a plan of
  • Identify needs and provide input for discharge planning and follow-up throughout the length of stay.

  • Implement a plan of care consistently, effectively, and cost-efficiently with a focus on resident-centered Identify rationale and anticipated outcomes for each nursing intervention.
  • Evaluate the effectiveness of the plan of care in conjunction with the interdisciplinary care team and make changes as needs are recognized.
  • Deliver and maintain optimum resident care and comfort by demonstrating knowledge and skills of current nursing practices. Transcribe physician and/or other health care professionals’ orders appropriately and accurately. Ask for clarification of questionable orders from the ordering physician and/or other health care professionals.
  • Maintain an updated knowledge base in demonstrating knowledge of policies governing medication administration and documentation.
  • Demonstrate working knowledge of infection control practices

Qualifications:

  • Must have a nurse apprentice license in the state of UT
  • Must be well-versed in interpersonal communications.
  • Must meet all local health regulations and pass the post-employment physical exam if required. This requirement also includes a criminal background investigation and reference inquiry.
  • Knowledge of nursing and medical practices and procedures, as well as laws, regulations, and guidelines that pertain to long-term care.
  • Effective interpersonal skills and the ability to work with an interdisciplinary care team.
  • Ability to plan, organize, develop, implement, and interpret the programs, goals, objectives, policies, and procedures that are necessary for providing quality care.
  • Basic understanding of computer technology, including email use.
  • Proven decision-making and analytical skills.
  • Able to relate positively, favorably, and cooperatively with others, including employees, residents, family members, and personnel of outside agencies and organizations.
  • Must be able to pass a drug screen and criminal background check.
