Sandhills Medical Foundation, Inc. is Hiring a Family Nurse Practitioner Near Lugoff, SC

I. Job Summary:

The Family Nurse Practitioner works as part of the clinic provider team performing a variety of direct patient services including assessment, diagnosis, implementation of treatment, evaluation, and monitoring the general patient management plan for clinic patients. The Family Nurse Practitioner preforms these functions under the general supervision of a licensed physician in a primary health care setting.

I. Supervisory Responsibilities:

  • May be asked to supervise Clinical Nurse or other members of the primary care team.

III. Duties & Responsibilities:

Patient Relations Responsibilities:

  • Obtains complete medical history from patient, and/or family, and/or previous medical record, etc.
  • Prescribes medications according to therapeutic goals, explains treatment to patients, and families
  • Teaches and counsels patients and families Staff Relations/Team Building Responsibilities:
  • Consults with physicians and other members of the health care team as necessary
  • Performs other necessary duties as required by Sandhills Medical Foundation, Inc. to meet the goal of providing primary health care services Clinical Duties:
  • Assesses, diagnoses, implements treatment, evaluates, and monitors patients' health status
  • Performs complete physical examinations on patients
  • Orders routine and diagnostic lab and x-ray studies according to standing orders
  • Diagnoses acute and chronic illness on the basis of clinical findings, laboratory and x-ray reports and/or the results of other diagnostic procedures according to approved protocols
  • Formulates, initiates and monitors patient management plans for patients assigned to the Family Nurse Practitioner, using protocols and/or consultation with a supervising physician
  • Refers patients requiring hospitalization or assessment and management of conditions to supervising physicians or appropriate specialist; follows patient’s progress with physician
  • Renders emergency care and treatment including cardiopulmonary resuscitation, starting IV fluids, and medications, sutures lacerations

Performance Improvement Responsibilities:

  • Maintains all patient records to comply with required standards, reviews records regularly and obtains physician counter signatures where necessary
  • Attends all medical staff meetings and participates in Sandhills Medical Foundation, Inc. ’ s Q/A program
  • Performs all other duties as assigned

IV. Required Education, Certifications, Skills & Abilities:

  • Graduation from an accredited school of nursing, a Nurse Practitioner program recognized by the Board of Nursing or from a program otherwise designated for Family Nurse Practitioners which has been approved by the State Board of Nursing
  • The ability to relate effectively to patients and providers
  • Must be qualified in Basic Life Support techniques

V. Additional Requirements:

  • Must possess valid driver's license
  • Must be willing to work with SMR EMR
  • Must be willing to work at different SMF locations if requested

VI. Training Requirements:

  • The Family Nurse Practitioner will be responsible for attending all mandatory training including evidenced based approaches to self-management support, proactively addressing the needs of patients and families in this community, cultural competency, effective communication, care coordination, OSHA, HIPAA, and HER, etc.

  • Ability to sit and/or stand for long periods of time
  • Constantly operates a computer and other office productivity machinery, such as a copy machine, computer printer and laboratory equipment
  • Computer literate in PC environment, including the ability to enter and access data relevant to SMF EMR system
  • Ability to physically write and visually read and evaluate patients
