Position Description

Payroll Title: HIV Outreach Coordinator

Functional Title: HIV Program Director

Summary

Under the supervision of the HIV Program Director, the HIV Outreach Coordinator (OC)/PrEP

Navigator will coordinate all outreach activities that relate to HIV prevention activities. The OC will keep daily track of program goals and objectives, support QI/QA tracking, prepare monthly reports, prevention outreach, campaigns, and intervention activities and volumes. The OC works with the HIV Care Coordinator to schedule counseling and testing visits and achieve established patient care goals.

The OC assists the Program Director in the development of social media events, site/venue strategy, culturally sensitive health education materials for both internal and public-facing audiences, public information postings and virtual-campaign events.

Essential Duties

  • Engage patients and link them to medical care for high risk HIV negative individuals
  • Assist with implementation of patient-centered care plans.
  • Conduct assessments for potential PrEP/PEP patients.
  • Identify and participate in outreach activities and relate to HIV prevention activities.
  • Attend community planning groups, advisory boards and committees.
  • Keep daily track of program goals and objectives, support QI/QA tracking, prepare monthly reports of prevention outreach, campaigns, and intervention activities and volumes.
  • Work with HIV Care Coordinator (CC) to schedule counseling and testing visits and to achieve established patient care goals.
  • Assist Program Director in the development of social media events, site/venue strategy, culturally sensitive health education materials for both internal and public facing audiences, public information postings and virtual campaigns/events
  • Assist with insurance eligibility check and applying uninsured patients towards financial assistance programs.
  • Perform related duties, including but not limited to outreach/prep navigating.

Requirements:

  • Medical Assistant Qualification.
  • Must become a Certified HIV Test Counselor within one year of commencing employment.
  • Minimum of one year experience in HIV prevention services.
  • One-year related experience.
  • Fluency in Spanish preferred
  • Full-time employee with schedule flexibility
  • Excellent interpersonal skills

Job Type: Full-time

Pay: $21.00 - $26.00 per hour

Expected hours: 40 per week
