San Diego Family Circle Adult Day Health Center, looking for a motivated social worker who enjoys working with a frail elderly population.Hours are great, 8:00 am to 5:00 pm Monday through Friday.

Working with frail seniors and disabled persons attending day health care

Must have one of the following:

• A master’s degree in Social work from an accredited school of social work
• Have a master’s degree in psychology, gerontology or counseling from and accredited school and has one year of experience providing social services in one or more of the fields of aging, health or long-term services
• Have a bachelor’s degree in social work from an accredited school with two years of experience providing social work services in one or more of the fields of aging, health or long-term services.

In addition, the position requires a health examination signed by a physician and a TB clearance no more than 12 months prior to employment and a current certificate of CPR and First Aid training.

Must be able to speak, read and write English fluently, bi lingual Spanish, a plus.

Duties include:

• Initial Assessments and reassessments of participants;
• Case management services;
• Daily documentation of services provided;
• Development of a plan of care;
• And other duties as required by the administrator and/or program director.
• Must work well within an interdisciplinary team, enjoy working with the elderly, have good communication skills and writing abilities.

Salary depends on experience.
