The Transitions Coach assists implementation of services and support to transitional aged youth (TAY) in accordance with the Center’s mission and values statement. The Transitions Coach assists the Transitions Coordinator, High School teachers, Therapy team, Academy Leadership, SDCC Academy staff, Residential ILS coordinator, and the TREC program, in implementing transitions services outlined in the Center’s successful Transitions Initiative, and in accordance with SDCC’s policies and procedures. The transitions coach will work with TAY clients in completing comprehensive assessments and individualized transitions plans. Provide career and college awareness coaching, training, mentorship, and on the job coaching to TAY youth and individuals with mental, emotional and/or behavioral challenges. Average caseload 8-12 youth. To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability required. The role involves both direct client contact and to much lesser degree indirect client-related duties. It is also necessary to undertake some administrative duties to ensure the smooth running of service provisions.
