Description

SUMMARY

The Transitions Coach assists implementation of services and support to transitional aged youth (TAY) in accordance with the Center’s mission and values statement. The Transitions Coach assists the Transitions Coordinator, High School teachers, Therapy team, Academy Leadership, SDCC Academy staff, Residential ILS coordinator, and the TREC program, in implementing transitions services outlined in the Center’s successful Transitions Initiative, and in accordance with SDCC’s policies and procedures. The transitions coach will work with TAY clients in completing comprehensive assessments and individualized transitions plans. Provide career and college awareness coaching, training, mentorship, and on the job coaching to TAY youth and individuals with mental, emotional and/or behavioral challenges. Average caseload 8-12 youth. To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability required. The role involves both direct client contact and to much lesser degree indirect client-related duties. It is also necessary to undertake some administrative duties to ensure the smooth running of service provisions. ESSENTIAL ROLES AND RESPONSIBILITIES

  • Work effectively as a member of a team in providing Transition's support to TAY youth.
  • Acts as a mentor and positive role model and assists in engaging youth in Successful Transitions Initiative.
  • Aligns with and helps support youth in navigating their Individualized Transitions Plan in accordance with their IEP.
  • Provide support to SDCC TAY students with career and college awareness, social and communication skills, work experiences, and independent living skills in a variety of campus and community environments.
  • Transport the TAY youth to community-based work experiences, appointments, and build relationships and coaching with community job sites and resources.
  • Assist in developing curriculum/activities and facilitate Transitions goals, Independent Living Skills and Life Skills for SDCC’s TAY students at the Academy.
  • Monitor and maintain detailed records of students’ progress.
  • Participates as the youth’s advocate at meetings.
  • Fulfills all required Academy and Center trainings.

Qualifications


MINIMUM QUALIFICATIONS

  • Valid California Driver’s License
  • Reliable transportation is required and a clean driving record
  • High School Diploma or GED
  • Experience working with adolescents with behavioral health problems in inpatient, residential, or outpatient facilities, wraparound and or special education settings.
  • Real life experiences navigating finding community resources, applying for jobs, resume writing, awareness of college life, and filling out student loan applications.
  • College-level courses in education, psychology or related field are desired but not required.
