We are seeking a skilled and detail-oriented MDS Nurse Coordinator to join our long-term care facility center. As the MDS Nurse Coordinator, you will be responsible for coordinating the Minimum Data Set (MDS) assessment process, ensuring accurate documentation, and collaborating with the interdisciplinary team to develop individualized care plans for our residents. The successful candidate will have strong clinical skills, knowledge of MDS guidelines, and a commitment to providing exceptional care to our residents.

Responsibilities:
- Coordinate and oversee the completion of the MDS assessment process for all residents.
- Ensure timely and accurate completion of MDS assessments and related documentation.
- Collaborate with the interdisciplinary team to develop care plans based on MDS assessments.
- Review medical records, conduct resident interviews, and perform clinical assessments for data gathering.
- Ensure compliance with MDS guidelines, state regulations, and company policies.
- Participate in care plan meetings and provide input on resident care needs and goals.
- Act as a resource to nursing staff regarding MDS documentation and coding.
- Maintain up-to-date knowledge of MDS requirements and updates.
Requirements:
- Licensed Practical Nurse (LPN) OR Registered Nurse (RN) with an active and unrestricted license.
- Minimum of 3 years of experience in MDS coordination in a long-term care or similar healthcare setting.
- Strong knowledge of MDS guidelines and regulations.
- Excellent communication and interpersonal skills.
- Detail-oriented with strong organizational and time management abilities.
