We are a non-profit Health Care and Hospice organization in Grand Junction, CO looking for a full-time Home Health Nurse Practitioner to join our team!

Our PACE program (Program of All-Inclusive Care for the Elderly), created in October 2021, helps seniors age well through social activities, education, and medical services. Seniors, their family members, and caregivers face many issues, including transportation to appointments, management of medications, coordination of medical care from different specialists, lack of social interaction, and the ability to stay alone at home. We understand the importance of a system that works together. PACE participants receive in-home care services and transportation to a PACE center for primary medical care, social and recreational activities, and other senior care services.
