Salmon Hospice is seeking a dedicated and compassionate Registered Nurse to coordinate and provide care to a caseload of hospice patients within the Natick/Framingham area. Working both independently and as part of a team, the Hospice RN Case Manager ensures that quality care is delivered in a patient-centered, holistic, and sensitive manner to meet the needs of end-of-life patients and their families. Qualifications:

• Licensure: Registered Nurse Licensed to practice in Massachusetts
• Experience: Minimum 1 year clinical experience
• Hospice experience preferred
