PHARMACY TECHNICIAN - MEDICATION RECONCILIATION TECH

Responsible for reviewing, clarifying, and documenting home medication lists of all patients admitted to the hospital. May also be required, under the supervision and direction of a pharmacist, to fill medication orders and perform related duties; and transport medications. The Medication Reconciliation Tech performs duties functioning in accordance with national and state standards, laws, and practices. Assists pharmacists as directed.

Minimum Education

· High school diploma or GED (or eight years of equivalent experience)

Minimum Experience

· Two years related experience preferred

· Retail pharmacy experience preferred

Required Registration/License/Certification

· Kansas State Board of Pharmacy registered technician preferred

· CPhT certification preferred
