Your Role With Us:

  • Interprets treatment goals, then makes recommendations regarding written medication orders or prescriptions ensuring that the medication treatment plan(s) are complete, understood by the patient as well as safe and compatible with institutional policy/protocols/best practice standards.
  • Participates in medication prescribing under Collaborative Practice Agreements (CDTM) with providers from Salem Health Medical group for chronic disease states and medically complex patients.
  • Provides collaborative drug therapy management (CDTM) services for patients being seen in the Emergency Department, and Ambulatory Care Clinics.
  • Physical assessment of patients related to disease states managed; Motivational interviewing and patient counseling.
  • Assisting in navigating third party payment plans (prior authorizations) as necessary based on supporting staff availability.
  • Provides drug monitoring service; Interprets policy and procedures for hospital and clinic staff.
  • Assesses and resolves medication therapy management educational needs of primary care providers including physicians, nurses, PAs, Nurse Practitioners, pharmacists, third party payers and other allied health professionals.

Background/Expertise:

  • Graduate of ACPE accredited college of pharmacy required (Pharm.D. degree preferred, B.S. degree acceptable with applicable work experience or PGY-1 residency program completion).
  • PGY-2 residency with ambulatory care focus ideal, PGY-1 residency requiredOR Four (4) years of previous ambulatory care clinical practice experience or clinical patient care practice, along with prior direct experience in patient/customer service interaction in-lieu of residency experience.
  • Current Oregon Pharmacist license or eligibility to apply for Oregon Pharmacist license.
  • BLS from the AHA required within 90 days of hire.
  • Appropriate Board Certification (BCPS, BCPAP, etc.) is required within two (2) years of accepting position.
  • Registration as active Preceptor per ASHP Residency guidelines.
