What you’ll be doing every day:

Your primary responsibility is to coordinate Social Services within your assigned neighborhood. This includes but is not limited to promoting and enhancing relationships between residents, their families and the support systems and resources inside or outside the community. 

What we are looking for:

You will have a Bachelor’s Degree in Social Work and at least one-year of case management experience within a Long Term Care, rehabilitation or healthcare community. You also must be able to demonstrate composure, customer focus, patience, and the ability to gain trust and establish rapport with residents that are living with mental health, alcohol/drug addiction, post-traumatic injuries, physical disabilities, and dementia.
