JOB DESCRIPTION
General Functions:
1. Works in coordination with the care team to optimize resident’s quality of life with the focus being that Saint Anne’s is the resident’s home.
2. Assist the team in maintaining Resident Rights.
3. Maintain the appropriate social services documentation. a. Obtain a social history upon admission. b. Progress notes c. MDS Documentation (BIMS, PHQ-9, Resident Preferences, etc) d. Write progress notes when interaction with the resident or family occurs e. Create and maintain Health Care Plans
4. Behavior Management a. Attend monthly Behavior Management Meeting with Behavior Management Nurse and
Pharmacist b. Work with Behavior Management Team on creating and implementing a behavior program for the necessary residents. c. In-service staff on resident’s behavior program and interventions. d. Review each resident’s behavior program and interventions quarterly. e. Work with the Behavior Management Nurse on having the necessary residents seen by the house psychologist or psychiatrist. Obtaining consent for those services. f. Follow up on any recommendations given by the psychologist or psychiatrist. g. Make Level 1 referrals to Park Center with mood or behavior changes occur.
5. Work with staff, family and residents on hospice services when appropriate. Make the hospice referral and insure proper services are being given to the resident and family.
6. Maintain inter-facility room move list and coordinate all in-house room moves.
7. Maintain the Missing Item and Grievance Log for you floor.
8. Complete, monitor and follow up on all necessary PAS paperwork (450B, e450B, Level 1’s).
9. Assist residents with obtaining personal items if family is unable to assist their loved one.
10. Obtain signatures for the Medicare Non-Coverage letters.
11. Coordinate and attend family meetings and health care plan conferences.
12. Attend staffing meetings as necessary: Admissions Meeting, Medicare Meeting, Apt/Rehab
Meeting, Safety Meeting, etc.
13. Participate in the Memorial Service.
14. Attend doctor’s appointments if family is unavailable and the unit secretary is unavailable.
15. Assist the admission’s team in giving facility tours if the admission team is unavailable.
16. Assist the team as needed with the Alzheimer’s Association Annual Walk.
17. Incorporate the resident’s advanced directives into their plan of care.
18. Have flexibility with your schedule per resident needs.
19. Assist with activities if needed.
20. Assist with the resident’s needs as they arise.
21. Conduct roommate conflict management as it arises.
22. Provide back up for creating Last Covered Day letters.
23. When I resident has an overnight leave to the hospital or other reason follow up with resident and/or family to explain the Saint Anne bed hold policy.
24. Follow up on any resident and family grievances.
Nursing Home Functions:
1. Conduct weekly one-on-one visits with all nursing home residents.
2. Assist residents and/or family with the Medicaid spend-down process.
3. In-house ancillary services: Obtain consent for podiatry, optometry, audiology and dental services.
At times assist resident in getting to these in-house appointments.
4. Occasionally coordinate discharge plans (see Rehab Functions for further information).
Rehab Functions:
1. Coordinate each residents appropriate discharge plan.
2. Coordinate and run discharge planning meetings with resident and/or family.
3. Make the appropriate community referrals to ensure a resident returns home safely. i.e. Home
Healthcare, Lifeline, Hospice, etc. Complete the necessary paperwork as follow up to those referrals.
4. Conduct discharge follow-up calls after a residents has returned home.
5. Assist resident and/or family with finding alternative placement when needed.
6. Work with Aging and In-Home Services with their Care Transitions Program.
7. When a resident discharges, put together a comprehensive discharge packet.
8. Conduct one-on-one visits with residents as necessary.
1. Coordinate and run a monthly support group for residents and families.
2. Conduct BIMS assessments as scheduled.
3. Be back-up to apartment admissions as necessary. Assist with tours and the admission process as needed.
4. Conduct one-on-one visits with residents as necessary.
5. Direct the Resident Council meeting in conjunction with the Resident Council President.
6. Ensure there is good communication between apartment residents and staff.
7. Facilitate good communication between the departments and units regarding apartment residents in rehab or the nursing home.
8. Conduct employee trainings and education for memory care and behavior management.
9. Implement a behavior management program when necessary.
10. Obtain a social history upon admission.
11. Assist residents with in-house and community resources.
12. Complete emergency PAS paperwork if a resident needs to move into LTC.

12. Ability to be flexible with your schedule from early morning to late evening to supervise and ensure residents routines are maintained.
13. Ensuring the team is identifying and providing floor activities of resident’s choice.
14. Possess problem solving skills.
Education and Formal Training:
1. Must hold appropriate State certification to perform the duties of a Social Worker in a nursing home in the State of Indiana, including post-secondary education. BSW or related degree is preferred.
2. Experience in long term care or working with the elderly for two plus years is preferred.
