Position SummaryUnder general supervision of Supervisor of Business Practices-Health Home and clinical supervision of the Chief of Health Services and Integrated Care, provides nursing services to consumers of Saginaw County Community Mental Health Authority (SCCMHA) in a variety of office and community-based settings, as assigned. This position serves adults and children with multiple physical health conditions, mental illness, substance use disorder(s) and/or developmental disabilities. Nursing services are provided according to the American Nurses Association and Michigan Nurses Association scope and standard of practice and the Michigan Public Health Code and Michigan Mental Health Code. This position will be knowledgeable about and actively support culturally competent recovery-based practices; person centered planning as a shared decision-making process with the individual, who defines his/her life goals and is assisted in developing a unique path toward those goals; and a trauma informed culture of safety to aid consumer in the recovery process.
Essential Duties And Responsiblities
  • Serves as a Registered Nurse for SCCMHA providing nursing services using the nursing process.
  • Coordinate delivery of healthcare services.
  • Employ strategies to promote health and a safe environment through health teaching and health promotion.
  • Incorporate knowledge of pharmacological, biological and complementary interventions with applied clinical skills to restore the consumer’s health and prevent further disability.
  • Provide, structure and maintain a safe, therapeutic, recovery-oriented environment in collaboration with healthcare consumers, families and other healthcare clinicians.
  • Use therapeutics and counseling interventions to assist healthcare consumers in their individual recovery journeys by improving and regaining their previous coping abilities, fostering mental health, and preventing mental disorders and disabilities.
  • Evaluate the consumers’ progress toward attainment of the expected outcomes.
  • Integrate ethical provisions in all areas of practice.
  • Serves as resource person for various activities within SCCMHA as well as contract agencies and other community agencies.
  • Will use a person-centered, evidence-based care delivery approach to ensure the coordination and continuity of health care as consumers transfer between different locations or different levels care. This includes assessing stability of consumers who have been discharged from an inpatient hospital setting, rehabilitation unit or skilled nursing facility and ensuring comprehensive, individualized transition planning.
  • Administer injectable medications, as ordered.
  • Provides nursing coverage where assigned.
  • Meet productivity and documentation standards established by the supervisor.
  • Assure that all billable services are documented and recorded within 5 business days of being provided.
  • Integrate evidence and research findings into practice.
  • Attain and maintain knowledge and competence that reflect current nursing practice, and each nursing role, duty and responsibility performed by RNs within the SCCMHA system.
  • Systematically enhance the quality and effectiveness of nursing practice by demonstrating quality through documentation, participation in quality improvement activities and adherence to quality requirements.
  • Communicate effectively in a variety of formats in all areas of practice.
  • Seek ways to advance nursing autonomy and accountability by providing appropriate quantity and level of nursing services that meet the consumer’s needs.
  • Collaborate with healthcare consumer, family, interprofessional health team, and others in the conduct of nursing practice.
  • Evaluate one’s own practice in relation to the professional practice standards and guidelines, relevant statutes, rules, and regulations.
  • Consider factors related to safety, efficacy, cost, and impact on practice in the planning and delivery of nursing services.
  • Practice nursing in an environmentally safe and healthy manner by reducing environmental health risks.
  • Maintain proficiency in the use of supplies, equipment, technology and tools necessary to provide nursing services.
  • Adheres to the mission, vision, core values and operating principles of SCCMHA at all times.
  • Must be compliant with Centers for Medicare & Medicaid Services (CMS) COVID-19 vaccine mandate.
Incidental Duties And Responsibilites
  • Attends meetings, in-service training, seminars, conferences, and workshops in order to sustain professional competency.
  • Reacts productively to change and handles other essential tasks as assigned.
  • Take the lead on special assignments as delegated.

Reporting RelationshipsReports to: Supervisor of Business Practices-Health Home and Chief of Health Services and Integrated Care.
Supervises:Provides clinical coaching and mentorship to other agency healthcare providers.
Working Conditions/EnvironmentWorks in an office and clinic-based setting with individuals with developmental disabilities, mental illness and/or children. Close contact with consumers who may have a communicable disease; and with consumers who may become hostile, aggressive, and/or potentially violent. Drives vehicle in all kinds of weather.
QualificationsEducation: Associate’s or bachelor’s degree in nursing (ADN or BSN) from an accredited nursing program.
Experience: One-year experience in medical-surgical nursing, community/public health nursing, or other fundamental nursing practice preferred. Community mental health experience preferred.
Licenses and Certifications:Valid Michigan Driver’s license with a good driving record.
A licensed registered nurse (RN).

  • Competent, across the lifespan, novice to expert nursing skills.
  • Ability to care for with persons who are developmentally disabled and/or mentally ill.
  • Ability to serve consumers in a pleasant, courteous manner, and communicate with individuals, families and colleagues effectively.
  • Ability to use tact and diplomacy with individuals from all social, economic and cultural backgrounds.
Physical/Mental Requirements
  • Hearing acuity to converse in person and on the telephone.
  • Visual acuity to observe consumers behavior, read and proofread documents and use EHR and other electronic devices.
  • Ability to stand, walk, bend, stoop kneel, push wheelchairs, and lift up to 25 pounds.
  • Manual dexterity to write and to operate standard office equipment (PC, Keyboard, Copy Machine, Fax Machine, etc.)
  • Strong interpersonal skills to interact with leadership, employees, consumers and the general public.
  • Analytical skills necessary to conduct research, analyze, and interpret complex data and identify and solve problems.
  • Ability to prioritize and manage job duties efficiently.
  • Ability to meet deadlines and work with large numbers of employees and/or consumers.
  • Must meet COVID-19 vaccine requirements.
