Overview:
We are seeking a dedicated and compassionate Memory Care Coordinator to join our team. As a Memory Care Coordinator, you will play a crucial role in managing and coordinating care for our residents. This position requires experience in senior care and memory care. If you are passionate about providing high-quality care and ensuring the well-being of our residents, we would love to hear from you.
Responsibilities:
- Coordinate and supervise the delivery of care services to residents
- Develop and implement care plans based on individual needs
- Monitor resident progress and make adjustments to care plans as necessary
- Provide guidance and support to caregivers and nursing staff
- Ensure compliance with resident rights and maintain confidentiality of resident information
- Collaborate with healthcare professionals to ensure optimal resident care
- Assist with medication administration and ensure proper documentation
- Conduct assessments and reassessments of residents' physical, emotional, and cognitive status
- Provide education and support to residents and their families regarding care options
Experience:
- Previous experience in nursing or senior care is required
- Knowledge of dementia care principles and practices is preferred
- Strong organizational skills with the ability to manage multiple tasks simultaneously
- Excellent communication skills, both written and verbal
- Ability to work effectively as part of a team
