Home Care On-call Manager, Thurs-Monday

Our understanding of aging related issues enables our team members to best serve the interests and needs of our clients and family members. Working in close collaboration with multidisciplinary teams of professionals, including Care Managers, Personal Care Attendant (PCA) Managers and the Recruiting team, the On Call Personnel Manager establishes caregiving teams, problem solving staffing and scheduling issues that arise, and actively and regularly communicates with bench PCAs. Under the supervision of the Director of Home Care, the On Call Personnel Manager plays a key role in ensuring the provision of high quality comprehensive and personalized Home Care services grounded in best practices and focused on quality of care and quality of life. 

This is an ideal role for a professional with experience in the field of aging and the provision of Home Care or other relevant personalized services; The Home Care On Call Personnel Manager is passionate about working with older adults and adults with disabilities and is a tenacious and creative problem-solver, able to bring optimism about what is possible for our client population, and a commitment to excellence.

RESPONSIBILITIES

  • Responds to and Handles All Incoming On-Call Calls - Responds to all home care after hours calls and effectively problem solves to ensure that all Personal Care Attendant and client needs are met. Communicating and collaborating with on-call Sage care management, client family members, client responsible parties, and appropriate professionals to ensure clients’ needs are met. To provide remote and direct emergency and on-going support for all after hours calls regarding all Sage home care clients and client schedules. 
  • Staffing (During Business Hours and After Hours) - Understands and is able to clearly describe the range of duties for which a Personal Care Attendant may be responsible in caring for individuals with a variety of physical, cognitive and mental health challenges.
  • Personnel Management Duties - Thinks creatively in proposing staffing and scheduling solutions to open staffing requests. Maintains accurate and current documentation of all open staffing requests, prioritizing effectively to ensure staffing of highest priority needs in a timely manner. Maintains up-to-date information on short and long term availability, including upcoming vacations and other planned absences, by meeting by phone, on average, once a week with each assigned, active and bench PCAs in order to check in and maintain rapport and to update spreadsheet 
  • Training Compliance- Uses the Online Training Compliance spreadsheet and workflow to reach out to PCAs that are coming up due or past due and set a deadline for training completion 
  • Recruiting Support - Messaging bench candidates, applicant tracking using applicant tracking system. Schedule recruiter phone interview and interview reminders.
 

REQUIREMENTS

  • B.A. and/or equivalent experience in the health or elder care fields (preferred) 
  • Experience providing direct services and solving problems with older adult population care or case management (preferred) 
  • Knowledge of local eldercare resources (preferred)
  • Availability some evenings and all weekend on call rotation
  • Proficiency with computer software programs: Google Drive, documents & spreadsheets, time management software, email, calendar and mobile applications
