SAFREST is Hiring a Weekend Hospice Registered Nurse Near Bridgeville, PA

We are seeking a dedicated and compassionate Weekend Hospice Registered Nurse to provide high-quality end-of-life care to patients and support to their families during the weekends. This position offers an opportunity to make a meaningful difference in the lives of patients facing terminal illness and their loved ones.

Key Responsibilities:

  1. Provide skilled nursing care to hospice patients in their homes or care facilities during weekend shifts.
  2. Perform comprehensive assessments of patients' physical, emotional, and spiritual needs, and develop individualized care plans in collaboration with the interdisciplinary team.
  3. Manage and coordinate all aspects of patient care, including pain and symptom management, medication administration, wound care, and end-of-life comfort measures.
  4. Educate patients and families about the disease process, treatment options, and available support services, fostering informed decision-making and autonomy.
  5. Communicate effectively with the interdisciplinary team, including physicians, social workers, chaplains, and other healthcare professionals, to ensure continuity of care and optimal outcomes.
  6. Document all patient care activities accurately and timely in electronic medical records, adhering to regulatory standards and guidelines.
  7. Provide emotional support and counseling to patients and families, addressing their psychosocial and spiritual needs with compassion and empathy.
  8. Advocate for patients' wishes and preferences regarding their care, ensuring that their values and beliefs are respected and honored.
  9. Collaborate with community resources and coordinate referrals for additional support services, such as home health, durable medical equipment, and hospice aides.
  10. Participate in interdisciplinary team meetings, case conferences, and in-services to discuss patient progress, develop care plans, and address any issues or concerns specific to weekend care.

Qualifications:

  • Valid Registered Nurse (RN) license in Pennsylvania.
  • Minimum of 1 year of experience in hospice, palliative care, or related healthcare setting.
  • Strong clinical assessment and critical thinking skills.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and make sound judgments in fast-paced and unpredictable environments.
  • Compassionate and empathetic demeanor.
  • Knowledge of Medicare/Medicaid regulations and hospice eligibility criteria.
  • Proficiency in electronic medical records (EMR) systems.
