Job Description: We are seeking a compassionate and dedicated Nurse Practitioner to join our team as a Home Health Care Nurse Practitioner. As a vital member of our home care team, you will be responsible for providing comprehensive medical care to patients within the comfort of their own homes. The ideal candidate will possess excellent clinical skills, strong communication abilities, and a commitment to delivering high-quality patient-centered care.

Key Responsibilities:

• Conduct thorough assessments of patients' health status and medical needs in the home setting.
• Develop and implement personalized care plans in collaboration with patients, their families, and other healthcare professionals.
• Perform clinical procedures such as wound care, medication management, and administration of treatments as needed.
• Educate patients and their families on disease management, medication compliance, and overall health promotion.
• Monitor patients' progress, evaluate treatment effectiveness, and adjust care plans accordingly.
• Coordinate with other healthcare team members to ensure continuity of care and optimal patient outcomes.
• Document all patient encounters, assessments, interventions, and outcomes accurately and in a timely manner.

Qualifications:

• Master's degree in Nursing from an accredited Nurse Practitioner program.
• Current unrestricted Nurse Practitioner license in Delaware.
• Certification as a Nurse Practitioner
• Minimum of 1 year of clinical experience, preferably in home care, primary, internal medicine, or community field.
• Strong clinical assessment and diagnostic skills.
• Excellent interpersonal and communication skills.
• Ability to work independently and as part of a multidisciplinary team.
• Proficiency in electronic medical record (EMR) systems.
• Valid driver's license and reliable transportation.
