Safer Foundation is Hiring a Treatment Clinician Near Chicago, IL

The Safer Foundation is a social service provider to individuals with arrest and conviction records. Through a full range of services, including case management, educational instruction and advocacy, we focus on preparing individuals for the world of work by helping them find and keep productive and meaningful employment.

The Treatment Clinician will be housed within the Safer Counseling and Wellness Center’s Clinical Center, with cross-functional functions and reporting to the Working | 4 | Peace employment program. This program works to engage individuals with arrest or conviction records living in communities affected by gun violence. This role Is responsible for all aspects of providing immediate crisis support, supporting mental health and substance use disorder evaluations, establishing referrals for the appropriate behavioral health level of care, monitoring clients’ progress, and ensuring continuity of care is not compromised. Will provide a range of clinical assessments and interventions and engage in internal and external staffing. The clinician will be offered extensive training to assess using IM CANS to provide clinical recommendations to improve the quality of life of returning residents. In addition, the clinician will be provided with professional development opportunities to support clinical professional growth and support for maintaining professional licensures and certifications.

This position will be responsible in

 a) conduct outreach and engagement activities at various community sites throughout the city,

 b) provide crisis intervention and case management to referred Working 4 Peace clients with immediate medical, mental health, substance use needs, or housing needs before engaging in employment or clinical services, and 

c) provide ongoing therapy and counseling to sss participants seeking social and emotional wellness support.

Essential Duties and Responsibilities:

Stakeholder Interface and Service

  • Maintain a caseload established by the clinical supervisor to provide clinical screening, assessment, and case management.
  • Conduct outreach to make contact, leveraging collateral contacts or other involved parties.
  • Complete an initial screening to identify/address immediate and short-term needs.
  • For all referrals, conduct initial screenings/assessments to determine immediate and short-term needs considering desired services, geography, client choice, insurance status, justice supervision status, and other considerations. 
  • Provide short-term and brief clinical services, including emotion regulation/stabilization and individual therapy for individuals experiencing high levels of distress and entering the workforce. 
  • Develop short-term and long-term recommendations/and treatment planning in adherence to a client’s reported needs.
  • Develop immediate safety plans for individuals at risk for violence. 
  • Provide crisis intervention, short-term counseling support, and ongoing clinical case management. 
  • Follow-up on the progress of initial service recommendations, adjusting when appropriate as immediate needs are met, and transition the client for ongoing case management with internal or external providers depending on the client’s needs and engagement in services.
  • Communicate with the clinical supervisor and cross-functional staff regarding client progress and identify needs to be addressed.
  • Using Salesforce, CareLogic, and/or other database(s), register clients into the system.
  • Provide individual and group therapy for substance use and mental health disorders.
  • Complete individualized treatment plans for each client within the guidelines set
  • Complete treatment plan reviews for each client at least monthly
  • Administer, interpret, and document urine analysis drug testing
  • Complete training and quality assurance activities within given timeframes.

Stakeholder Outreach and Recruitment

  • Under the direction of the clinical supervisor, seek out, research, negotiate and establish new relationships with other behavioral health agencies that welcome and are responsive to the needs of the justice-impacted population.
  • Coordinate and conduct integrated staffings with internal programmatic staff and external providers with external partners while obtaining the necessary consent and maintaining confidentiality by HIPAA and, when applicable, 42 CFR Part 2 or other relevant confidentially laws.

Tracking, Document, and Reporting

  • As clients stabilize, refer to internal programs to begin enrollment and initiation into employment programming. Schedule intake appointments directly or with the appropriate program staff. Ensure warm handoffs are made, and activity is updated in Salesforce for other involved staff.
  • Maintain compliance with HIPAA, 42 CFR Part 2, and other privacy and confidentiality laws and regulations concerning clients’ protected information.
  • Attend internal and external multi-disciplinary staffing to discuss collective individuals on the team’s caseload.
  • Referrals are made and follow-up with appointment dates and kept appointments.
  • All contractual requirements are completed accurately, entirely, and on time.
  • All Safer standards are adhered to, including productivity standards.

Required Knowledge, Skills, and Abilities:

  • Knowledge of mental health counseling, individual and group therapy, and psychoeducational treatments.
  • Knowledge and willingness to learn IM CANS assessments and interpretation of such assessments.
  • Knowledge of completing ASAM bio-psycho-social assessments and interpretation of such assessments. 
  • Knowledge of crisis evaluation and intervention. 
  • Knowledge of treatment plan development and implementation per IL Medicaid Rule 132 and SUPR 2060.
  • Knowledge of human, emotional, social, psychological, and environmental challenges and interventions to alleviate them.
  • Knowledge of the techniques for observing and assessing behavior
  • Knowledge of the primary mental health professional discipline, principles, and practice
  • Knowledge of the characteristics and aspects of mental and emotional disturbances and developmental disabilities.
  • Ability to work with other professional disciplines. Ability to make independent clinical judgments.
  • Ability to communicate effectively with others, including organizing, evaluating, and presenting information verbally, in writing, and speaking before groups.
  • Ability to develop a therapeutic relationship with patients. 
  • Familiarity with Medicaid Rule 132 and SUPR Rule 2060
  • Knowledge of standard concepts, theories, issues, and best practices concerning Chicago neighborhood trends, individuals impacted by the justice system, and community re-entry. 
  • Computer/Technology Literate: Knows how to use the internet, computer programs, and applications to obtain information.
  • Ability to troubleshoot and problem-solve.
  • Knowledge of standard concepts, theories, issues, and best practices concerning reentry
  • Knowledge of computer-based and web-based applications (Microsoft Office, Internet Explorer, Google Chrome, Mozilla Firefox, etc.)
  • Ability to troubleshoot, problem-solve
  • Ability to represent Safer Foundation professionally while also being able to develop a rapport with clients from diverse backgrounds

  • Knowledgeable about Medicaid in Illinois as well as other public benefits

  • Knowledgeable about the Application for Benefits Eligibility (ABE) website and Manage My Case function
  • Knowledgeable about housing resources and eligibility requirements for various housing types
  • Skilled in active listening and conveying information to others effectively
  • Skilled in Social Perceptiveness (Being aware of others’ reactions and understanding why they react as they do)
  • Skilled in Critical Thinking, judgment, and decision-making
  • Skilled in de-escalation techniques and conflict resolution

Education and Experience:

  • Master’s degree in social work, Counseling, or other human service-related field required.
  • Licensed or licensed eligible within two years of hire LPC/LSW required.
  • A minimum of 2 years experience working in community behavioral health settings is required.
  • A minimum of 1-2 years providing crisis intervention services is preferred social services to individuals at risk of violence, and gun violence and domestic abuse. 
  • At least two years of experience working as a case manager in a social service setting.
  • Must understand utilizing DSM-5 criteria for diagnosing mental health and substance use conditions. 
  • Experience working with individuals impacted by the justice system preferred.
  • Experience in referring clients to a network of community-based providers and resources.
  • Experience in substance use/mental health or care coordination.
  • Knowledge of group behavior and dynamics and societal trends and influences.
  • Proficient in utilizing multiple internet-based platforms preferred.
  • Proficient in Microsoft Office programs preferred (Microsoft Word, Teams, Excel, etc.).
  • Proficient in completing clinical documentation per IL Rule 132 and SUPR 2060 compliance standards preferred.
  • Knows how to leverage the internet and computer applications to obtain information.
  • In-depth practical knowledge of HIPAA and 42 CFR Part 2.

Licenses and or Certifications:

 LPC, LSW, LCSW, LCSW, and CADC certification/license or any other permits that satisfy 77 Ill. Adm. Code 2060.309 governing substance use services in Illinois is required.
