SafeHouse Outreach is Hiring a Diversion and RRH Program Manager Near Atlanta, GA

JOB DESCRIPTION

Summary/Objective:

The Diversion and RRH Program Managerwill be responsible for overseeing the Bridge Rapid Rehousing and Diversion Programs under the direction of the Director of Programs. The diversion and rapid rehousing programs entail street outreach, diversion document readiness, and case management of those experiencing homelessness and extreme poverty.

Essential Functions

· Establishes relationships and communicates with clients, caseworkers, family members, and staff.

· Identifies and seeks resolution for potential problems arising with all involved parties.

· Strong verbal skills, to communicate effectively with staff, volunteers, and community groups.

Supervision and Leadership:

· Lead and mentor a team of case managers, navigators, and/or coordinators providing guidance, support, and regular grant performance feedback.

· Conduct regular team meetings and goal-setting sessions as it pertains to program deliverables.

· Foster a collaborative and positive work environment, promoting professional growth and development within the team.

Program/Reporting and Management:

· Under supervision, implement program policies, procedures, and protocols in line with best practices and regulatory requirements.

· Monitor program activities, ensuring they are consistent with established standards of service delivery and quality assurance.

· Make recommendations for strategies to ensure program outcomes are met.

· Conduct monthly/as needed housing inspections for program participants to ensure program compliance.

· Coordinate and manage maintenance requests.

· Oversee the collection, analysis, and reporting of program-related data for internal and external reporting purposes.

· Ensure accurate and timely reporting to funding agencies and stakeholders.

· Maintain up-to-date client files and program statistics, including, but not limited to, client data and grant-related documentation.

· Conduct scheduled client record audits to ensure compliance with schedule of service contact.

· Review landlord/tenant dwelling leases and supporting documentation for program compliance.

· Investigate complaints, document and take appropriate action in such case, per federal, state, and local policies and regulations.

· Ensure compliance with program protocols, standards, and regulatory requirements.

· Generate reports on counseling activities, outcomes, and program effectiveness for management and regulatory purposes.

· Makes recommendations for program improvements.

· Develops professional relationships with other peers in the field.

· Attend meetings and collaborate with external partners

Participant Support and Case Management:

· Oversee case management services provided to program participants and provide case management as needed, ensuring individualized support plans align with client needs.

· Monitor client progress, assess needs, and make adjustments to service plans as necessary to promote housing stability and personal growth.

· Assess housing barriers of individuals and families experiencing homelessness to determine housing and service needs.

· Develop a housing procurement, financial, and self-sufficiency case management plan with clients. This shall include intake interviews to determine client’s needs, goals, and eligibility.

· Collaborate and coordinate educational, vocational, employment, and career development and exploration activities.

· Provide mediation and advocacy with landlords on the client’s behalf to develop a workable plan to obtain and or maintain housing.

· Create and maintain consistent communication channels, both verbal and written, between several parties (i.e. tenant, landlord, referral source, collaborating agencies, debtors and creditors).

· Provide pro-active follow-up home visits to ensure stability and further progress towards self-sufficiency; this includes support, advocacy, reducing isolation, listening, problem solving, and identification of resources to assist with reintegration of participants in the community.

· Apply knowledge of residential lease contracts to educate clients of their rights and responsibilities.

· Transport clients as deemed necessary to housing and job searches and occasional visits to relevant social service agencies.

· Assist participants in locating and securing permanent supportive housing of their choice.

· Collect and report program data, including but not limited to HMIS reporting and funders’ required data.

· Provide accurate, complete, and timely data, and reports to Program Manager as needed; Record data for program outcomes; and ensure client files are complete, accurate and up to date.

· Ensure client confidentiality according to SafeHouse Outreach policies and procedures

· Maintains all client documentation in the electronic record.

Education/Experience:

· College Degree

· 3 years of experience.

· Experience in more than one of the following disciplines: office administration, case management, customer service, and operations.

· Valid Georgia driver's license.

Job Type: Full-time

Pay: $45,000.00 - $50,000.00 per year
