FLEXIBLE SCHEDULE

Qualifications:

QMHP or qualified mental health professional-community services ~ A staff member who is credentialed as a QMHP-CS who has demonstrated and documented competency in the work to be performed and has:

  • A minimum of a bachelor's degree from an accredited college or university with a minimum number of hours that is equivalent to a major in psychology, social work, medicine, nursing, rehabilitation, counseling, sociology, human growth and development, physician assistant, gerontology, special education, educational psychology, early childhood education, or early childhood intervention
  • Service Area is West Columbia, TX (77486) - Travel is required, therefore must live within a 30 mile radius (Damon, Angleton, Lake Jackson, Richwood, Van Vleck, Bay City, Freeport).

General Responsibilities:The workers is responsible for providing medical necessary services to the client in accordance with the treatment plan that is designed to meet the mental health, educational, vocational, residential, financial, social and other needs of the consumer.

  • Travel to client location (home, school, office or community setting) and provide structured face-to-face scheduled sessions to client based on the treatment plans
  • Ensure that the majority of mental health services are primarily rendered in the community and ensure that limited services remain clinic based.
  • Maintain a client caseload of 2 to 10 clients
  • Attend supervision, training, and job shadowing sessions
  • Serve as an intricate member of the SP team to ensure continuity of care and supports the services 24 hours a day, seven days per week as necessary
  • Work collaboratively with and provide ongoing updates to the clinical team including psychiatrist as needed
  • Completes and attest to completion of all mandatory training as identified in TRR
  •  Ability to work efficiently and effectively both individually and as part of a team
  • Ability to appropriately accept feedback through the supervision process - thus displaying the willingness to learn, grow, and improve
  • Submit notes and reports timely (clinical notes, service log, mileage tracker, etc.)
  • Well organized and adaptable
  • Demonstrates good insight and judgment
  • Effective conflict resolution skills
  • Provide skill building and case management services to clients
  • Effective oral and written communication skills
  • Ability to work with children and family to deliver skills through approved curriculum
  • Ability to support the agency's culture, growth, and success through communication, accountability, and positive
