Multidisciplinary Team Coordinator - Child Advocacy Center

Summary:

The MDT Coordinator is responsible for coordinating efforts between Law Enforcement (NYPD), Child Protective Services (ACS), Prosecution (DA Offices and Corporation Counsel) Medical Personnel (Child Abuse Pediatricians and Nurse Practitioners) and Mental Health/Victim Advocacy (Safe Horizon) to provide a comprehensive multi-agency response to child victims of alleged sexual and/or severe physical abuse and their impacted family members. Additionally, this position is responsible for monitoring the progress and outcome of all cases throughout a family’s involvement with the CAC. 

Reporting directly to the CAC Senior Director, the MDT Coordinator is the only position in the entire CAC facility that is responsible for facilitating, navigating and coordinating the complex multidisciplinary team response of six on-site partner agencies and their respective staff. Capitalizing on the position’s critical importance in the CAC model and structure, critical goals of the MDT Coordinator’s complex role include:

  • Coordinate and facilitate case specific MDT meetings
  • Learn and integrate a trauma-informed, client-centered approach, using an anti-oppressive lens, into your work with clients.
  • Activate and coordinate an immediate CAC MDT response for each new case referred to the CAC
  • Mobilize MDT members assigned to each new case to facilitate the sharing of critical intake information to initiate the investigation MDT response
  • Ensure that timely pre-forensic and post-forensic interview MDT meetings occur with team members assigned to each case
  • Ensure that MDT members are keeping client families informed on the status of the investigation and response while they are at the CAC (i.e. follow-up appointments, court dates)
  • Facilitate multi-disciplinary team case planning and case review meetings in the absence of the Senior Director (often discussing between 30-60 cases during each meeting including on-site and off-site MDT members assigned to each case) and/or support the Senior Director in meeting facilitation
  • Maintain a system for identifying all cases to be discussed at MDT meetings and tracking progress and outcomes of cases

Provide consultation and coordination

  • Work in collaboration with the CAC medical team to ensure the timely provision of on-site medical examinations for child victims are scheduled
  • Be available for case consultation for community partners requesting an MDT meeting to determine appropriateness and next steps
  • Connect community partners with other professionals involved in their cases
  • Advise community partners of available resources for children and their families

Work directly with child victims and involved family members

  • Meet with children and families (at child interviews, hearings, etc.) to introduce them to the system and available services
  • Work in collaboration with Safe Horizon CAC staff to advocate on behalf of the child victim and involved caregivers to address case management needs including the utilization of Safe Horizon’s Emergency Client Assistance Program (ECAP) and/or Project Debby;
  • Answer questions and provide support for families while at the CAC
  • Respond to parents and families who present to the CAC in crisis
  • Serve as the point person for the New York State Office of Victim Services (OVS) by overseeing the proper completion and submission of crime victims’ benefits applications to OVS on behalf of children & families and following up with OVS to ensure submission of missing data, and to confirm benefit denials and awards

Qualifications:

  • Ability to influence, navigate and facilitate multidisciplinary team communication and actions
  • Excellent organizational and time management skills required
  • Strong communication, interpersonal, problem-solving and crisis management skills required;
  • Expected to complete additional training to obtain specific knowledge on multidisciplinary team coordination and facilitation
  • Two (2) years of professional experience working with children and families who have either experienced abuse or impacted by trauma and/or two (2) years of professional experience in working with the criminal justice or child welfare system required.
  • Proficiency in excel, and willingness to gain expertise in CMS
  • Bachelor’s degree in social work, counseling, human services or related field or equivalent training or experience;

Hiring Range: 25.27 - 28.43 Hourly
