Qualification/Responsibilities:

• Must possess a current, unencumbered, active license to practice as a Registered Nurse in the state of employment AND/OR
• Must possess, as a minimum, a Nursing Degree from an accredited college or university, or be a graduate of an approved LPN/LVN program
• Preferably 1-year experience in working the RAI process
• Preferably 1-year experience in MDS Coordinator
• Conduct and Coordinate the development and completion and the resident assessment (MDS) in accordance with current regulations and guidelines
• Must be experienced in PDPM, MDS coding, insurance updates, care planning 
• Long-Term Care Experience Required
• Assist in completion of patient discharge
• Auditing the resident assessment (MDS) and care plan for completeness and accuracy.
• Develop, implement and maintain a quality assurance program resident care plan.

Shift & Wage:

• Full-Time Opportunity Available
• Shift Needed: 8:00am-4:00pm, Monday-Friday
• Wage: Starting at $30.00 per hour
