Purpose: The Access to Care Coordinator will be a peer support specialist who is a qualified professional. This individual will be responsible for coordinating access to care for all the patients in the behavioral health emergency room of our partnering agency and those referred to SHYAS for services. The Access to Care Coordinator will provide supervision to all patient access reps/MOA staff. In addition, the access coordinator will manage design of treatment access protocols employed within the agency; and ensure appropriate access to care and continuum of care for all people served and supported. T

Qualifications

Is a certified peer support specialist with the ability to read, write and understand how to follow instructions AND

  • Has no violent criminal record or substantiated findings of abuse or neglect listed on the North Carolina Health Care Personnel Registry AND
  • Has a bachelor degree, Masters Degree or Equivalent (License Clinician preferred) AND
  • Is a Qualified professional with knowledge of and experience with the target populations served AND
  • Has two years of management experience

Responsibilities

1. Manage ALL intakes:

    *Document each step in the access to care database. *Coordinate intake and Assessment for all referrals *Ensure all beds are full at all times. Beds should not be empty over 48 hours. *Monitor ADT feeds and assign each patient to a protocol and care manager. *Ensure all new TCM consumers are input into the EMR and assigned to a care manager *Ensure the consumer is assigned to the appropriate service and clinician/team in the EMR system. *Complete client specific training with assigned counselor/clinician when assigning service to begin monitoring and documenting the submission of initial and ongoing Treatment Team Meeting/Treatment Planning and outcome data management (Baseline, RAS, IMR Clinician/client rating scale) *Ensure the patient begins receiving care/treatment within 14 days of referral. *Inform referral source via email of the Access to Care timeline. Monitor and ensure submission of ongoing monthly DSS/Probation reports, NC TOPPS. *Complete 30-day & 90-day follow up to ensure the patient has accessed care.

2. Monitor and Manage consumer database/electronic health record:

    *Ensure that each consumer is entered upon referral. *Ensure consumer profile is complete in its entirety upon receipt of assessment. *Ensure assessment date, NC TOPP dates, PCP dates and are entered and the dates of the Target Pops are entered along with the due date of each revision/update. * Input and update client requirements. Ensure that all client requirements and authorizations are entered and updated as needed.

3. Compliance:

    * Complete all access to care monthly, quarterly, and annual reports as required by the county/MCO and the SHYAS administration. *Complete 30-day & 90-day service reviews for all new patients.

4. Provider Relations:

    *Attend assigned community provider meetings and actively participate on those subcommittees. *Maintain positive working relationships within the communities served, including individuals, families, staff, monitoring and licensing agencies, organizations, and patrons.

5. Policy & Procedure:

    *Manage implementation of treatment access protocols employed within the agency. *Ensure staff follow intake, assessment, medication management, and treatment protocols. * Ensure that staffing schedules and daily operations meet the demands of the consumers and services. Arrange for supervision of program when away from the program.

Available shifts and compensation: We have available shifts all days of the week. Compensation is $18.00 - $23.00/hour.
