• Part Time or Per Diem RNs Case Managers (flexible schedule)

Job Requirements:

• · Comprehensive knowledge and skill in current nursing practice.

• · Knowledge of the concept of team and primary care nursing and the implementation of both models of care delivery.

• · Knowledge of the OASIS process.

• · Knowledge of the community resources.

• · Ability to make meaningful observations, evaluations and reports.

• · Ability to recognize the needs and concerns of patients, caregivers, and the multidisciplinary team to maintain therapeutic relationships.

• · Ability to provide skilled nursing care based on scientific principles, the nursing process, basic behavioral concepts, and intelligent professional judgment.

• · Valid driver’s license.
