• Brother Bonaventure Extended Care & Rehabilitation Center is a not-for-profit skilled nursing center affiliated with Trinitas Regional Medical Center. We are a 124-bed facility designed to provide continuous care and support for individuals who require long-term care or sub-acute care before returning home. Our facility, in conjunction with Trinitas, is committed to improving the health status of the people and communities we serve. We strive to promote independence and improve functional capacities to enhance the quality of life of our residents.
• Brother Bonaventure is licensed by the State of New Jersey. We are approved by Medicare and Medicaid as well as Managed Care. We offer 24-hour licensed nursing care; direct access to full hospital support services including the Wound Care Center, Dialysis Center, ophthalmology, podiatry, dental and psychiatric services, physical, occupational and speech therapies, Social Work services, therapeutic recreation and pet therapy; Pastoral Care programs and Religious services; laundry services; and professional hairdresser, barber and specialized dietary services.

• Brother Bonaventure Extended Care & Rehabilitation Center provides 24-hour licensed nursing care under the supervision of our Medical Director. Our Medical Director, in cooperation with our staff physicians, provides excellent care in managing the overall health of our residents.
• Our registered and licensed practical nurses, along with our certified nursing aides and licensed PT, OT, and Speech Therapists, assist long-term and sub-acute patients in recovering from and managing various acute medical and orthopedic conditions. Special and individualized care is provided for all residents.
• Brother Bonaventure Extended Care & Rehabilitation Center utilizes our full-service Wound Care and Dialysis Centers, located at Trinitas' Williamson Street Campus. We also work in cooperation with our on-site Behavioral Health Services and Psychiatric Emergency Department, for those patients that may require immediate psychiatric attention and/or placement.

Job Summary:

• A Supervisor will assist in planning and implementing nursing care services as per NJ Dept, of Health, State and Federal regulations. Provides for continuity of care and services for each resident under the direction of the Nurse Manager Charge Nurse, and/or Physician. Assesses resident needs on admission and as needed on a timely basis. Performs treatments and administers medication with physician s order. Responsible to follow standards of practice, policies and procedures set by facility, State, and Federal standards. Supervision of ancillary personnel on unit to ensure quality of care services, Participates in performance appraisals. Functions as Charge Nurse if situation requires. Assists ancillary staff with direct patient care activities as needed bed baths, incontinent care, Feeding. Responsible for nursing documentation on each resident including, but not limited to, care plans, summaries, nurse s notes. Assists Charge Nurse in recording and collecting data needed for continuity of care diabetic testing, intake and output, Physician s overdue visits. Care and maintenance of nursing equipment and supplies. Communicates with nursing and ancillary personnel to coordinate planning intervention processes and procedures as needed. Observes and reports on patient status to Nurse Manager and/or Physician. Assists Nurse Manager Charge Nurse in staff replacement within guidelines and policies set by Administration. Attends and participates in meetings related to resident care and unit management. Assists Charge Nurse in coordinating and implementing interdisciplinary care plans to meet resident needs. Participates in change of shift reports. Assists in admission, transfer, and discharge of residents. Reinforces basic nursing care concepts to ancillary nursing personnel. Identification of high-risk residents who would develop pressure sores and timely implementation of preventive and supportive strategies for wound care. Accurate identification and assessments of residents prone to develop sentinel events such as dehydration, fecal impaction, fracture. Assists in implementing and maintaining preventive strategies for infection control and follows infection control policies and procedures. Reinforces teaching efforts of Nurse Manager Charge Nurse and or In Service Education Department to all nursing personnel. Assist the nurse manager in MDS care planning process to ensure continuation of care. Ensure and assist in order transcription and monthly recaps. Assumes responsibility for own personal educational and developmental needs,

Educational Skills Required Degrees, Certifications, License,

• Licensed in New Jersey as Practical Nurse having graduated from an accredited school of practical nursing.

• Work Experience Required.
• Must have good interpersonal and communication skills.
• Must have certification in cardiopulmonary certification.
• Prefer at least one year of experience in long-term care nursing, but not required.
