Job Overview:
The Clinical Manager, In-Patient and Post-Transplant Services, practices as an independent care provider in collaboration with the transplant physicians, including the Chief of Transplant, primarily in the inpatient Renal & Pancreas Transplant Unit and Transplant Short Stay Unit (TSSU) and out-patient Pre- and Post-Transplant setting.

Essential Functions:
The Clinical Manager collaborates with Transplant Administrative Leadership in coordinating and overseeing in-patient care, patient transition of care to an outpatient transplant clinic, as well as APP care in the outpatient transplant TSSU and Clinic. They have direct supervisory responsibility for Transplant APPs. Their practice is characterized by comprehensive health assessments and management of patients with acute and/or chronic renal disease. The Nurse Practioner shall have clinical responsibilities inclusive of transplant candidates, living donors, post-transplant patients and patients enrolled in clinical research trials. Obtains patient histories and performs physical examinations. Writes admitting orders, routine orders and daily progress notes as indicated. Acts as liaison between Cooperman Barnabas and all referring dialysis centers, community agencies and referring physicians. Ensures continuity of care through effective communication and collaboration with the multidisciplinary team in all areas of the transplant department. Uses an understanding of the principles of growth and development to assess each patient’s age specific needs and provides age specific treatment. Serves as a resource for nursing and ancillary staff with regard to patient and disease management. Facilitate practices that are in compliance with JCAHO requirements and transplant regulatory requirements.
