Job Overview:
The Case Coordinator works collaboratively with patients, physicians, family and providers of healthcare to implement a plan of care which meets the individual s needs. This is coordinated by assessing, planning, implementing, advocating, monitoring, and evaluating options and services, to meet the individuals health care needs.

Qualifications:
Required:

• Must be a graduate from an accredited school of nursing
• 5 years acute hospital experience
• Additional experience required based upon specific department needs

Preferred:

• BA/BS/BSN Degree in Nursing
• Completion of Clinical experience in NLN accredited school

Certifications and Licenses Required:

• Current Registered Nurse License in the state of New Jersey
• Basic Life Support (BLS) from the American Heart Association
• National certification preferred

Scheduling Requirements:

• Full-Time
• Day shift
• 40 hours weekly
