What You'll Do: As a Hospice RN, your mission will be to deliver compassionate and personalized patient care in alignment with physician's orders. Your responsibilities will include:
  • Regularly assesses and reassesses the nursing needs of the Hospice patient. Competently implements care needed for comfort and palliation of symptoms
  • Provides direct client care and works closely with case manager in implementing care.
  • Performs admission procedure as per RVNAHealth policies using electronic health record in a timely manner with accurate documentation. Develops a plan of care consistent with the physician’s plan of treatment and patient/ family wishes.
  • Coordinates the implementation of the plan of care for patients residing in SNF, ALF, hospital contract bed or personal residence.
  • Provides dietary counseling.
  • Provides Hospice nursing services, treatments and preventive procedures.
  • Initiates nursing procedures appropriate for the patient’s Hospice care and safety.
  • Observes signs and symptoms and reports to the physician and IDG members any unexpected changes in the patient’s physical or emotional condition.
  • Teaches, supervises and counsels the Hospice patient, family members and facility staff about providing care for the client.
Experience:
  • Graduation from an accredited school of nursing; BSN preferred
  • Currently licensed to practice as a Registered Nurse in the State of Connecticut.
  • A minimum of two years of nursing experience and active patient contact preferred.
  • Prior Hospice experience not required, we will train you.
  • Ability to work independently, make accurate, and at times, quick judgements.
  • Able to drive in all types of weather.
