RUSH Health is Hiring a Pharmacist (Retail) Part Time Near Chicago, IL

Work Type: Part Time (Total FTE between 0.5 and 0.89)

Shift: Shift 1

Work Schedule: 8 Hr (8:00:00 AM - 4:00:00 PM)

Summary: The Pharmacist is responsible for assuring the effective and efficient provision of pharmaceutical care services. Perform all duties necessary to provide for the effective daily operation of departmental services and programs. The Pharmacist is proactively involved in safe, rational and cost-effective therapeutic decision making. In the course of performing his/her duties, the Pharmacist is responsible for the activities of pharmacy residents, students and supportive personnel as assigned in all practice areas. The Pharmacist reports directly to the immediate supervisor of the assigned practice area. Exemplifies the Rush mission, vision and values and acts in accordance with Rush policies and procedures.

Responsibilities: 1. Evaluates physicians’ orders for therapeutic appropriateness, proper dosing, drug interactions, allergic reactions, route, labs, contraindications and rate of administration and therapeutic redundancies. 2. Verifies orders in Epic in a timely manner and if necessary, clarifies orders through direct and timely contact with prescriber or designated practitioner. 3. Workflow may include evaluation and/or Epic order entry of cytotoxic drug therapy orders. The order evaluation may be performed on new orders or as a profile review of a target patient population. 4. A profile review would include (but not be limited to) a focus on: high risk medication review for appropriateness in dose, indication and duration of treatment; reduction of unnecessary medications; drug-drug or drug-food interaction evaluation; changes in renal and/or hepatic function that would impact medication dosing; use of trigger medications that would reflect an adverse event such as a adverse drug reaction or medication error. 5. Provides patient-specific pharmacotherapy recommendations and drug information to the medical and nursing staff. Provides general and/or patient specific drug information as required to other health professionals. When appropriate, makes recommendations for cost-effective alternatives to medication orders. In-services to nursing units as needed, to communicate changes in Rush formulary or other specific drug use issues. 6. Performs medication admission and allergy histories and discharge counseling per the Pharmacy Department’s direction (may include all patients, those receiving high risk medications or another identified subset of patients). 7. Appropriately documents in Epic all interventions, checking of patient’s own medication, vancomycin/aminoglycoside consultation, patient education (i.e. warfarin teaching) and any communication related to admission medication histories or discharge counseling. 8. Performs daily review of pharmacy reports: IV to PO, CrCl report, pharmacokinetic monitoring and any other drug or patient-focused report that may be required. 9. Attends CPR emergencies; provides coverage for other pharmacists involved in these emergencies. Pharmacist is expected to be BLS, ACLS and where appropriate, PALS certified. (See Department of Pharmacy policy on adult and pediatric code coverage). 10. Conducts monthly nursing unit inspections of medication storage areas as assigned. 11. Implements all drug use policy and formulary decisions of the Pharmacy and Therapeutics Committee. Oversees therapeutic substitutions as appropriate. 12. Participates in Unusual Occurrence, UO (ADR and medication error) reporting program. 13. Provides clear and specific handoff communications to next provider of care whenever necessary. This may be the next pharmacist on shift, or other healthcare professionals, such as when patients are being discharged. 14. Identify medication safety issues and takes steps to make corrections, or brings system change suggestions to the attention of the appropriate personnel. 15. Participate in quality assurance projects, drug utilization evaluations and formulary monograph development as assigned. 16. Rotation from a unit-based position through the central pharmacy will be expected to happen on rotation with other clinical staff pharmacists. This will include the checking of pyxis, patient specific medication supplies, IV and TPN production 17. Provides general supervision for technical support in their assigned operational area. Monitors and coordinates workflow with established delivery schedules.

Other information: Required Job Qualifications: •Bachelor of Science degree in pharmacy and/or PharmD. •Licensed or eligible for licensure in Illinois. •BLS, ACLS and where appropriate, PALS certified. •Effective verbal, interpersonal, and written communication skills. •Previous hospital pharmacy experience and a PGY1 residency. •Strong work ethic with demonstrated leadership activities or potential for developing leadership behavior. •Must be able to work collaboratively with other healthcare professionals, including within the Department of Pharmacy. •Continually support and participate in the development of the Pharmacy Department’s goals and objectives. Candidates must be comfortable with change that is associated with these efforts •Demonstrates leadership in performance of expected responsibilities. •Maintains currency in the professional and clinical literature of the profession. •Maintains competency in unique aspects of drug therapy for the patients care is provided for. Demonstrates a thorough understanding of clinical monitoring activities. •Demonstrates the ability to use standard texts, online drug information resources and other sources of information and apply it to general patient care questions. •Effectively and respectfully communicates with patients, pharmacists, nurses and physicians. Represents the face of the Department of Pharmacy in a professional manner during every interaction with our customers. •Maintains working knowledge of medical center and departmental policies and procedures, including safety issues, inventory control and distribution, and infection control. •Participates in educational seminars presented by residents in the Department. •Required to attend medical center and/or departmental inservice or continuing education programs. •Completes required age/disease state specific competencies. Completes medical center required annual competencies or educational programming. •Assists in the training of new employees, support personnel, residents, and students. Precepts pharmacy students on hospital and/or clinical rotations, consistent with academic credentials and clinical experience. •Takes responsibility for personal and professional growth within assigned position in the Department of Pharmacy. •Maintains active Illinois licensing status to practice. •Encouraged to pursue board certification (BPS). •Ensures compliance with accreditation standards and with federal, state and local regulations. •Maintains Rush ICARE values (innovation, collaboration, accountability, respect and excellence). Works cooperatively with others to use appropriate systems strengths, knowledge and cooperation to improve performance. •Responds to the department’s Disaster Plan as prescribed by medical center policy. •Reviews policies and procedures when requested. •Belongs to local/state/national professional pharmacy organizations; attends meetings. •Active participation in professional organizations is encouraged. •Participates on other projects as assigned or as pertains to the pharmacy area.
