Summary: The Patient Care Navigator: Concierge Registered Nurse (RN) identifies patterns of patient responses and can use past experiences to implement solutions for current situations. The Patient Care Navigator: Concierge RN is a competent nurse who has mastered technical skills and utilizes evidence-based interventions to provide care. The Patient Care Navigator: Concierge RN collaborates with other members of the interdisciplinary team. This person exemplifies the RUSH mission, vision, and values and acts in accordance with RUSH policies and procedures.
Responsibilities: Responsibilities Evidence-Based Intervention Utilizes system wide resources to support decision making and action. Recognizes patterns of problems in a specific patient population that require investigation and initiates actions to address. Communicates current evidence-based information with peers. Relationships and Caring Provide individualized nursing care to clients based on their unique healthcare needs, preferences, and lifestyle. Establishes a caring, therapeutic relationship with patients and families through practice communication, acknowledgement of patient and family perceptions of health and illness and mutual goal setting for optimal outcomes. Critical Thinking Utilizes competent assessment skills to holistically evaluate the physical and psychosocial needs of patients and families. Demonstrates the ability to prioritize care for complex patients based on knowledge acquired from previous clinical experiences. Recognizes changes in patient status and consistently responds effectively. Technical Expertise Provides safe, competent patient care within the framework of the concierge medicine program, utilizing an array of technologies to meet individual patient needs. Individualizes teaching plans based on a thorough assessment to meet the learning needs of patients and families. Leadership Collaborate with healthcare professionals, including physicians, specialists, and allied healthcare providers, to develop and implement personalized care plans for clients. Delegates effectively and consistently to optimize patient outcomes. Demonstrates accountability for own professional practice, including progress toward achievement of annual goals. Coordinates and provides sensitive medical and primary nursing care for patients in the concierge medicine program.
Other information: Job Requirements Current State of Illinois registered nurse license. Maintain current basic life support (BLS) certification. Earned nursing degree at the baccalaureate level (BSN) or higher. Two years registered nurse experience required. Ability to support the Concierge Medicine program including after-hours responsibilities related to patient care coordination and scheduling. Preferred Qualifications Analytical ability. Communication skills are required in order to lead and teach.
