POSITION SUMMARY
Provides relevant nursing care in a primary care setting. Nursing care is provided under the guidance and supervision of the Senior Site Supervisor and/or Director of Nursing and/or Assistant Director of Nursing. Responsibilities include collection of data, administration of medications and treatments, and documentation of care provided.

1. Improve patient outcomes, and deliver quality care within practice areas as assigned. Ensure all clinical care is delivered in a manner that meets or exceeds goals and expectations for clinical outcomes, quality assurance standards, and patient satisfaction.
2. Ensures timely and accurate collection of patient data, and makes observations on assigned patients to provide supportive data in the patient assessment.
3. Promotes attainment of specified goals through health education to patients/families.
4. Provides service in a manner that is appropriate for the patient’s age; demonstrates knowledge and skills necessary to meet the patient’s physical, psychosocial, educational and safety needs.
5. Ensure all patient charts and related documentation are maintained current and relevant at all time.

6. Ensure all care and nursing practices are conducted consistent within current, relevant standards, as well as state and local laws and regulations, Bureau of Primary Health Care and related professional standards.
7. Demonstrate competency in practice and knowledge of current standards of practice. Maintains credentials and performs current nursing practice expectations within established guidelines.
8. Provides care in a manner that respects the patient’s rights and choices in a multi-cultural setting.

9. Develops and maintains favorable internal relationships, partnerships with co-workers, nursing staff, physicians and business office staff.

10. Ensure all actions, job performance, personal conduct and communications represent the organization in a highly professional manner at all times.

PRIMARY TASKS & DUTIES
1. Provide direct care to patients as assigned and qualified to perform, (e.g. history, life cycle, etc.)
2. Assess and review care to ensure maximum patient outcomes.
3. Provide counsel, support, advice, and support to patients and their families.
4. Complete all patient charting, and ensure all patient records are current and complete
5. Attend seminars and maintain all licensure requirements for continuing education and best practices.
6. Participates in quality strategies to evaluate compliance with standards and to identify opportunities to improve patient outcomes.
7. Prepare clinic rooms to assure they are stocked with necessary supplies, equipment, etc.
8. Administer immunizations according to VFC CDC and RHSC guidelines.
9. Cleans and disinfect equipment and supplies according to established guidelines.
10. Notify patient of results, appointments, medications, etc. as directed by the physician.
11. Initiates intravenous fluids per physician orders if certified in IV therapy.
