Rubicon Programs is Hiring a Health and Wellness Case Manager Near Richmond, CA

The Health and Wellness Case Manager/Wellness Coach develops a trusting, encouraging relationship to facilitate participants’ progress along their individualized wellness plan. Being healthy and well is central to vocational success and economic progress. Supportive relationships are critical to navigating complex systems and being intentional around self-care and healthy behaviors. In addition to the responsibilities and competencies detailed below, innovation, initiative and a tolerance for ambiguity will therefore be critical competencies for success.

This is a full-time, non-exempt position reporting to the Wellness Manager, and compensation is commensurate with experience.

ESSENTIAL RESPONSIBILITIES

  • Coaching: Direct supports might include 1 on 1 session to assess participant needs and collaborate with the participant to develop and follow a wellness plan; ensure participant accesses Wellness Services; and motivate the participant to achieve wellness goals and coach participant in making progress towards wellness goals.
  • Group Facilitation: Facilitate participant workshops and groups and/or classes on wellness related topics and activities at Richmond and Antioch sites.
  • Systems Navigation: Assist with scheduling and preparation for appointments, transportation to appointments, accompany participants to appointment. Navigate public systems to obtain information, services and advocate on behalf of participant, as needed.
  • Data Entry: Data capture and entry at all points of the case management process and provide accurate and timely reporting per agency standards.

COMPETENCIES

  • Work experience in the areas of Motivational Interviewing, Trauma Informed Care, coaching; working knowledge of best practices for assisting economically disadvantaged and/or other under-represented populations.
  • Teamwork, interpersonal flexibility, excellent organizational and communication skills, and the ability to inspire others; effective teaching and or group facilitation ability.
  • Strong knowledge of services (social services, health care) and networks in East and West Contra Costa County.
  • Group facilitation skills and abilities; computer and internet proficiency.
  • This position requires the incumbent to travel efficiently throughout the East Bay. If they opt to meet these travel requirements by driving, proof of a valid and current driver license, current auto insurance, and an acceptable Motor Vehicle Report are required.

COMPENSATION AND BENEFITS

  • $25-28 per hour, depending on experience.
