Responsibilities: - Provide personal care assistance to individuals in their homes or residential facilities - Assist with activities of daily living, including bathing, dressing, grooming, and toileting - Administer medications as prescribed by healthcare professionals - Monitor and record vital signs and report any changes or concerns to the appropriate personnel - Assist with meal preparation and feeding, ensuring dietary restrictions and preferences are followed - Provide companionship and emotional support to individuals - Assist with light housekeeping tasks, such as cleaning and organizing living spaces - Follow all resident rights policies and procedures to ensure the well-being and safety of individuals - Collaborate with healthcare professionals, social workers, and other team members to provide comprehensive care Qualifications: - First aid and CPR certification preferred - Experience in patient care or home care setting is a plus - Knowledge of medication administration procedures - Familiarity with working with individuals with autism or other developmental disabilities - Ability to lift and transfer individuals as needed - Understanding of hospice care principles and practices is beneficial - Strong communication skills and ability to work effectively in a team environment Please note that this job description is not exhaustive and additional duties may be assigned as needed.
