Royal Palm Beach Health and Rehabilitation Center is a 120-bed Skilled Nursing Facility. Located in the Village of Royal Palm Beach. We offer short-term rehabilitation as well a long term and hospice care services. Our qualified staff is committed to providing patients with an individualized plan of care designed to meet each individual's physical, social, spiritual and emotional needs.

We are urgently hiring for a MDS/PPS Coordinator to join out team.

Summary:

Under the direction of the Director of Nursing Services, the MDS Coordinator is a responsible for implementing the federally mandated assessment tool to identify needs/concerns of the residents and to coordinate plan of care to assist residents toward goals or optimal wellness based on the highest practicable level of functioning.

Job Duties:

Maintain a working knowledge of an ensure compliance with all government regulations related to the RAI/Care Plan process and where applicable with the case mix reimbursement program.

Conduct care plan meetings with responsible staff members, interdisciplinary team members, residents and family member where applicable.

Coordinate a comprehensive assessment (including MDS, RAPS, triggers narrative RAP summary, care plan) on admissions, within one year of the most recent assessment, or whenever there is "significant change" in resident's status. Coordinate quarterly assessment as required.

Where applicable, transmit assessment date to state agencies, monitor, and correct Medicaid roaster and coordinate assessment to maximize Medicaid case mix reimbursement.

Coordinate and ensure timely completion of the RAI/care plan process according to federal guidelines.

Qualifications:

Must hold a current and valid RN License

Related experience at a level necessary to accomplish this position.
