What does a Rouse Admissions Case Manager do?

• Review patient records, assessing for patient’s appropriateness and medical stability for admission to Skilled Nursing facility.
• Knowledge of ICD-10 diagnoses coding relating to long term care.
• Conduct on-site assessments/reviews in the community for potential admissions.
• Analyze and solve customer problems by preparing and providing solutions and following up on referrals to ensure customer satisfaction and promote positive outcomes.
• Identify and recommend special equipment needs and precautions to assure safe continuum of care and make recommendations regarding cost-effective and appropriate admission orders.
• Must have the ability to respond to admission referrals when the Admissions office is closed (nights, weekends and holidays).
• Other duties as assigned.

What are the requirements to be an Admissions Case Manager at the Rouse?

• Valid LPN, PTA or COTA license or Bachelor’s or Master’s Degree in Social Work required.
• Minimum of three (3) years of experience in long-term care with working knowledge of insurance, medications, Medicare and Medicaid regulations, as well as skilled nursing care and intermediate care levels required.
• Valid Driver’s License required.
