Responsibilities

• Participates in the development of individual service plans for new residents and ongoing review/updating of individual service plans in response to changes in resident needs.
• Participates in the completion of resident assessments.
• Ensures resident charts are current and organized and required documentation is completed.
• Assists with admissions and discharges to ensure coordination of services.
• Performs routine reviews of physician orders for accuracy.
• Functions as liaison with families, physicians, and related healthcare agencies regarding care/ health and wellness issues for residents.
• Models teamwork and demonstrates the ability to interact positively with a caring, nurturing, and professional attitude.
• Other duties as assigned.

Qualifications

• Current RMA or RN/LPN license
• Current adult CPR certification
• Excellent customer service and interpersonal skills
• Knowledge of or willingness and ability to learn computer programs.
• UAI/ISP certification or willingness to obtain certification within 60 days of hire.
