General Statement of Duties: Manage the care and treatment program of a case load of residents. Provide psychosocial care to the residents of Rose Hill and support them in achieving and maintaining their highest level of independence. This is accomplished by supporting their individualized goals through our work team activities, group therapies, and engagement in social activities. Case managers demonstrate the ability to engage residents in a welcoming, hopeful, empathetic manner regardless of illness or phase of recovery and use principles of trauma informed care and a willingness to support and assist residents with trauma related issues.

Responsibilities and Duties:

• Case Management provide overall management and direction for the care of residents.
• Develop and implement a Person-Centered Plan of Service.
• Communicate with families, significant others, mental health professionals, and appropriate health care providers to facilitate the resident’s treatment program.
• Assist residents in holding outside employment by treatment plan.
• Facilitate group therapy on a weekly on-going basis to the residents.
• Complete required assessments on assigned residents and update as scheduled.
• Maintain an accurate and thorough medical record on each resident.
• Assist in the medical and pharmacological program for the residents.
• Communicate with the Medical Director, attending psychiatrist and other clinical team members regarding the status of the residents.
• Provide shared on call clinical coverage after normal working hours.
• Provide crisis intervention as appropriate.

Qualifications:

• Master’s degree in social work or other mental health profession with appropriate state licensure.
• Substantial experience in working with individuals with a serious mental illness in either a hospital or community setting.
