General Purpose: Responsibleto assist daily social service tasks, as described below and any additionalassignments received from director. Qualified individual must posses a caringand interest in assisting the elderly and disabled population, and assistfamilies as needed. Must be able to communicate effectively. Perform dutiesusing independent judgment and provide for resident’s social, emotional, andpsychological needs.

Social Service Functions:

• Develop a socialhistory for all residents
• Activelyrepresent social services in interdisciplinary care conference meetings whendirector requests or when unavailable, responsible for documentation andattendance record
• Document progressnotes as necessary for individual residents
• Complete MDSassessments as assigned per MDS coordinators
• Deliverinvitations and encourage residents and families to attend care conferences
• Serve as eachresident’s advocate, and support each resident’s individual concerns andchoices
• Educate/reportany events in violation of resident rights observed by staff, family, orvisitors
• Ensure roomchanges are completed properly
• Document residentand new roommate adjustment for three days
• Assist residentsand families with adjustment to facility
• Assist eachdepartment as able and promote teamwork
• Understand andabide by the Corporate Standards of Conduct policies
• Completion of weeklyGuardian Angel program
• Keep grievancebook up to date and assure all grievances are resolved and given to Hendrik
• Relias trainingcompleted monthly as assigned
• Ancillaryservices
