This is multi-location position, with the expectation to be flexible in terms of being able to travel between locations within Chautauqua County.

Your position will play an essential role in the care of our most vulnerable population, our seniors. You will work directly with our occupational therapy and nursing teams to provide exceptional therapy, support, and care for each resident we serve.
As a Per DiemPhysical Therapy Assistants (PTA), you will be responsible under the supervision of a physical therapist, to administers complex active and passive therapeutic exercise to motivate and assist residents in learning and improving mobility.
Essential Duties:
Provides appropriate treatment, as determined by the Physical Therapist, to help residents maintain or attain the highest level of function possible within the confines of his/her physical abilities.
Recommends to the Physical Therapist appropriate treatment, preventative measures, adaptive equipment, and/or durable medical equipment to prevent decline in residents’ function.
Provides appropriate resident and caregiver in-service education to help residents maintain or attain the highest level of function possible within the confines of his/her physical abilities.
Monitors Medicare eligibility requirements including, initiation of coverage, length of coverage, minutes treatment was delivered and termination of coverage. Communicates all information to appropriate personnel.
Complete documentation, including but not limited to progress notes, discharge summaries, screens, MDS assessments, and recommendations for resident care plans in a timely manner.
Responsible for staff training, quality improvement, auditing medical records, etc..
Demonstrates knowledge of Physical Therapy procedures and equipment providing for safety and maximum resident benefit.
Follows Plan of Care and performs therapeutic interventions as directed by the Physical
QUALIFICATIONS:
Graduate of accredited Physical Therapy Assistant program.
Current NYS Physical Therapy Assistant licensure. Must maintain license.
