SUMMARY OF RESPONSIBILITIES

The Director of Nursing (DON) for Supportive Living at Sophia Snow Place has a pivotal role in providing leadership for high quality, person-centered and age-friendly care for each resident and their family members. Under the supervision of the Executive Director, the DON manages the overall operations of the clinical care and management for residents in SSP Supportive Living in accordance with SSP policies and procedures, professional standards of nursing care and government regulations -- ensuring excellence and ongoing improvement in the delivery of optimal resident care, while promoting of well-being and respect for dignity of each person.

The DON is responsible for providing direct care and ongoing assessment of the health status of residents who need supportive care and supervising clinical staff in carrying out individualized care plans to meet the needs of each resident. Other key responsibilities include providing, directing, and monitoring care plans to meet the changing needs of residents, including ongoing clinical assessments, medication administration, monitoring nutrition and dietary needs and reviewing the support needed for activities of daily living. A crucial aspect of this role is to provide leadership for creating a respectful and supportive work environment and culture to optimize each staff member’s contribution to the mission and goals of SSP Supportive Living. The DON serves as a liaison to family members, physicians and additional care team members and emergency response teams (EMTs).

As the clinical leader, the individual has an important role in promoting safety, maintaining quality assurance, identifying opportunities for improvement, and facilitating performance improvement activities. The DON also plays an active role in interdepartmental meetings and collaborations, compliance efforts and strategic planning with the Executive Director, SSP CEO and Board members.

GENERAL QUALIFICATIONS:

· Knowledgeable about the care provided to residents in Level IV Rest Homes and relevant standards of care, Federal and State (DPH) regulatory and certification requirements.

· Demonstrates expert clinical assessment skills and competencies to provide expert care for residents with a variety of clinical needs and cognitive capabilities.

· Provides emotional support and compassion with sensitivity to issues of aging, disability (including cognitive decline) and management of chronic illnesses – for both residents and family members.

· Communicates effectively and demonstrates skills in collaboration, team building and conflict resolution.

· Exhibits strong leadership, organizational and management qualities – ability to set goals and objectives and engage all staff in efforts to ensure that expectations are met.

· Collaborates effectively with physicians and care team members at SSP and within the community.

SPECIFIC QUALIFICATIONS:

· Bachelor of Science degree in Nursing (BSN) from an accredited School of Nursing is required; Master of Science in Nursing (MSN) is preferred.

· Must maintain a current RN license to practice nursing in Massachusetts.

· 3-5 yrs. direct patient care experience; minimum of one year in geriatric nursing; experience in a post-acute care setting preferred (skilled nursing facility, assisted living, home health care, and nursing or rest home)

· Minimum of one year in a leadership and administrative position, with experience supervising incensed practical nurses (LPNs), clinical nursing assistants (CNAs) and nursing aides

· Must be certified and competent in C.P.R. (cardiopulmonary resuscitation) and A.E.D (automated external defibrillator).

· Must be available to be on-call after regular business hours.

SPECIFIC AREAS OF RESPONSIBILITY:

· Completes comprehensive assessments for all new potential residents in the community for possible placement at Sophia Snow House.

· Completes comprehensive resident assessments every 6 months thereafter (and as needed if the clinical status of a resident changes) and ensures that staff are informed of all significant changes of condition and revised care plans. Ensures that each resident’s goals are clearly identified, and that appropriate interventions and timelines are established and communicated to the care team.

· Initiatives treatments and care for all SL residents based on the comprehensive assessments and coordinates patient care activities with other members of the health care team and SSP colleagues. Facilitates effective communication of residents’ needs and issues to the interdisciplinary team and fosters teamwork.

· Directs and coordinates the care given by Certified Nursing Assistants, (CNAs), Nursing Aides, Licensed Practical Nurses (LPNs), and Licensed Clinical Social Worker (LCSW).

· Work closely with physicians and other community health care providers to ensure optimal care of each resident.

· Directs and evaluates the supervision and/or supervision of medication administration, initiating corrective action if there are medication errors, adverse events or inaccurate documentation of medications. Ensures review of medication cart, resident EHR and paper file.

· In partnership with the Executive Director and or CEO, conducts investigations of residents, family, and employee concerns, such as but not limited to Incident Reports, Employee Injury reports, resident concern, or one-on-one meetings.

· Assists in identifying root cause and recommends corrective action plan.

· Prepares necessary and/or required reports according to DPH and SSH guidelines.

· Participates in the hiring, orientation and retention of LPNs, CNAs, Responsible Persons, and Nursing Aides

· Conducts staff development and continuing education training as needed.

· Supervises, mentors, and conducts and documents yearly evaluation clinical staff (initiating disciplinary action if warranted)

· Motivates, enables, and empowers staff to jointly plan and deliver exemplary care to residents and family members. Uses appropriate interpersonal skills to guide others to accomplish objectives, facilitates teamwork, participation, and cooperation.

· Organize and directs clinical staff, creating scheduling plans and distribution of LPNs, Responsible Persons (RPs), CNAs, nursing aides on all shifts. Delegates work so team members have the necessary direction, authority, and skills to complete assignments.

· Develop and maintain assignment sheets for Supportive Living direct care staff based on the resident service plans.

· Serves as a clinical resource to safely manage admission, discharge, and transfers in accordance with DPH regulations. Reviews admission and re-admission orders.

· Ensures that all DPH guidelines, regulations, and professional care standards for Level IV Rest Homes are followed, documented.

· Develops and ensures compliance with SSP nursing policies, procedures, and job descriptions.

· Meets regularly with the Executive Director in overseeing the SL budget, designs and manages continuous quality improvement (CQI) initiatives and to provide oversight for SL operations and problem solving. Reviews all assigned quality reports/activities that affect quality of care and quality of life for residents. Identifies gaps in existing work processes and participates in resolution.

· Develop staff to achieve quality outcomes.

· Participates in interdepartmental meetings as directed.

· As outlined in the strategic plan, communicates departmental goals and objectives, leads improvement efforts and measures clinical outcomes and administrative compliance.
