Job Overview:
We are seeking a compassionate and organized Care Coordinator to join our team. As a Care Coordinator, you will be responsible for coordinating and overseeing the care of individuals in need of assistance. This position requires experience in meal planning, social work, dementia care, direct support, and behavior management.

Duties:
- Coordinate and schedule care services for resident
- Conduct assessments to determine client needs and develop care plans
- Provide support and guidance to caregivers
- Monitor client progress and make adjustments to care plans as needed
- Collaborate with healthcare professionals and community resources

- Oversee medications, med cart and med techs
- Assist with meal planning
- Provide social work support to clients and their families
- Implement behavior management strategies for clients with challenging behaviors
- Ensure compliance with all regulations and standards related to assisted living and home care systems

- Communicate changes or concerns with residents families, staff and Executive Director

Experience:
- Previous experience in caregiving or a related field is required
- Knowledge of dementia care techniques and best practices
- Familiarity with behavior management strategies
- Strong organizational skills and attention to detail
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a team

Job Type: Full-time

Pay: $19.01 - $22.90 per hour
