These values are the core of who we are as an organization and shared among all affiliated facilities Responsibilities: Conduct comprehensive evaluations of patients to determine their physical therapy needs and develop individualized treatment plans. Provide evidence-based outpatient physical therapy services to patients with various musculoskeletal and neurological conditions. Implement therapeutic exercises, manual therapy techniques, and modalities to improve patients’ mobility, strength, and function. Monitor patients’ progress closely and adjust treatment plans as needed to ensure optimal outcomes. Educate patients and their families on therapeutic exercises and home exercise programs to facilitate continued progress outside of therapy sessions. Collaborate with interdisciplinary team members, including physicians, nurses, and other therapists, to coordinate patient care and achieve treatment goals. Maintain accurate and timely documentation of patient evaluations, progress notes, and treatment plans in accordance with facility policies and regulatory requirements. Qualifications: Graduate of an accredited Physical Therapy program. Current state licensure as a Physical Therapist. Pay rates are competitive and based on various factors.
