Rock County is Hiring an Outpatient Psychotherapist- Human Services Professional III Near WI 53546, WI

Provides person-centered, trauma informed services to adults with complex and/or co-occurring behavioral health needs in an outpatient setting: Provides assessment, psychotherapy, treatment planning, case management, and discharge planning both in person and via tele-health. Provides individual and group therapy services. Documents care provided. Collaborates with internal and external colleagues. Participates in clinical and administrative meetings. Supports and promotes the Mission, Vision and Guiding Principles of the Human Services Department. Performs in a manner consistent with the HSD Professional Standards and “Commitment to My Co-Workers.

70% Provides person-centered behavioral health care services for adult clients

  • Develop therapeutic relationships with clients
  • Create person-centered treatment plans and updates
  • Assess psychosocial issues including social support, barriers to treatment, trauma, and substance use.
  • Provide individual, family, and/or group psychotherapy
  • Assist clients in crisis including assessing risk and developing safety plans
  • Coordinate client care and discharge planning as needed
  • Refer for psychiatry and other services as indicated

10% Ensure program and medical record compliance 

  • Follow all departmental as well as divisional policies and procedures
  • Document client care in accordance with policies, procedures, and regulatory guidelines

8% Participates in all clinical and administrative activities as required

  • Participate in case review meetings
  • Participate in staff meetings
  • Participate in meetings with supervisor as needed

7% Coordinate client care

  • Communicate regularly with client, other professionals, and client's support network as indicated
  • Advocate for client as needed

5% Will perform other duties as assigned

  • Knowledge of the principles and practices of behavioral health care and its application, including person-centered recovery plans, trauma-informed care, co-occurring disorders, and stages of change.
  • Knowledge of laws, regulations, and practices pertaining to federal and state human service and behavioral health care programs
  • Knowledge of assessment and treatment of mental illness and substance use disorders
  • Knowledge of psychotropic medications, their main effects, side effects, and abuse potential.
  • Knowledge of psycho-social rehabilitation services
  • Ability to maintain accurate records and prepare clear and concise reports
  • Ability to establish and maintain effective working relationships within the agency and community.
  • Ability to understand and carry out verbal and written instructions
  • Ability to take initiative, work independently, and seek out supervision as needed
  • Ability to manage one's time and emotions in an environment of rapidly changing priorities
  • Excellent computer skills related to electronic record keeping, email, and internet

 Masters degree or higher from an accredited college or university with a major in Psychology, Social Work, Professional Counseling, Marriage and Family Therapy or related field
Wisconsin Licensed Professional Counselor (LPC), Licensed Marriage and Family Therapist (LMFT), or Licensed Clinical Social Worker (LCSW) license required.
 
Must possess and maintain a current unrestricted (not due to corrective lenses) driver's license and auto insurance pursuant to the Rock County Administrative Policy and Procedure Policy 5.02 and 5.27.
