SUMMARY:
The Nurse Educator is responsible for identifying, facilitating and evaluating educational activities for nursing and clinical staff. This position provides opportunities to promote life-long learning for employees and lends to safe, quality, patient care.
ElderONE, an affiliate of Rochester Regional Health, is a Program of All-Inclusive Care for the Elderly (PACE). Servicing Monroe, Ontario and Wayne counties, ElderONE is an innovative alternative to long-term nursing home placement. As the area’s only PACE program, ElderONE represents the future of senior health care. This long-term care program provides the entire continuum of medical care and support services to seniors 55 and older with chronic care needs, allowing them to maintain their independence in the comfort of their home without compromising their health or safety.
