As our team continues to grow, we are excited to welcome a licensed Physical Therapist. As the Physical Therapist you will provide therapy services to patients impaired by physical and/or cognitive dysfunction, including assessment, treatment planning and implementation of treatment plans. Please note this is a dual role, you will be responsible for providing care in the Skilled Nursing Facility and Home Care visits.
Responsibilities:
Conducts physical therapy evaluations to determine patient physical status using standardized tests, and measures to assess patient’s strength, motor development, joint mobility, sensory perception, balance, functional capacity, circulatory efficiency, and tolerance to physical activities.
Develops and implements physical therapy treatment plans using strengthening, modalities and retraining strategies consistent with the professional and ethical standards of the APTA in order to improve and maintain function and to reach patients’ highest potential for independence.
Checks, instructs, assigns and reviews the work of PTAs and technicians as assigned.
Assess effects of treatment on an ongoing basis and modifying the patient’s plan to achieve maximum benefit.
Evaluates, fits and adjusts prosthetic/orthotic devices along with prosthetist/orthotist.
Confers with physician and other practitioners to obtain additional patient information, suggest revision in treatment program and integrate physical therapy treatment with other aspects of patient’s health care.
Provides documentation to maintain quality of the medical record as it relates to accuracy, continuity of care, reimbursement guidelines, and agency requirements.
Participates, as requested, in patient and family care conferences.
Educate and instructs patient and family/caregiver(s) in treatment procedures to be continued when not in therapy or as a home exercise program.
Participates in Quality Assurance activities as directed by Agency Administrator, and/or site-specific Director of Therapy.
Promotes current standards of clinical practice through participation in internal and external continuing education, in-service programs and certifications sponsored by the agency.
Participates in new program development to expand the clinical services of the rehabilitation agency.
Required Qualifications:
Successful completion of an accredited Physical Therapy program
Licensure as required by the State of Michigan
Rotates to other clinic locations to meet patient care needs and other duties as assigned.
Ability to exercise professional judgement and initiative when analyzing problems and recommending solutions.
Ability to react effectively to the needs of patients and their families.
Physical Ability to bend, position, transfer and lift patients weighing from 80 pounds to over 300 pounds, without restriction, utilizing lifting devices clinically recommended for the patient.
